- SF 1444 RFQ and provisions Statement of Work This requirement is for Animal Caretaker Services on a Government Owned Kennl. Attendants must be able to pass a limited background investigation and a drug screen. If a site visit is needed before a quote can be provided, please contact me immediately to make arrangements. Customs and Border Protection has a requirement for dog animal caretaker services at Otay Mesa, CA. The current requirement is for eighty dogs; however, that number can fluctuate if dogs come and go for temporary assignments. Please see attached Statement of Work for details of requirement. This is a request for quotes for kennel attendant services on an as-needed, daily basis. Please provide a price quote in terms of hourly rate per attendant. Also, express the change in rates for weekend/holiday pay. Award of this requirement will be subject to availability of fiscal year 2010 funds. While the Service Contract Act does apply, there is no specific labor category for kennel attendant. Vendors must complete and return the SF 1444 attached, to be reviewed by Department of Labor. Award will be made on the basis of lowest-priced and technically acceptable. Awardee MUST register in Central Contractor Registration (www.ccr.gov) and receive payment via Electronic Funds Transfer (EFT) as a condition of the contract. Registration in CCR requires a valid Dun & Bradstreet number. Please complete and SIGN the price quote form attached, and return with this form. YOU MUST ALSO COMPLETE AND SIGN Blocks 12, 13, 14, 15 and 16 on page one of the SF 18 form. Quotes that are incomplete, unsigned, or late will not be considered. All quotes must be RECEIVED by the Contract Specialist no later than October 7th, at 12:00 P.M. NOON Eastern Standard Time (Indianapolis).
