- The VA Butler Healthcare, 325 New Castle Road, Butler, PA has a requirement for diagnostic radiology services to be performed at the contractor's place of business in support of and in proximity to the VA Community Based Outpatient Clinic (CBOC) located in Hermitage, PA in Mercer County. Selected offeror will provide the facility, staffing, equipment and supplies required to perform a wide variety of diagnostic radiology examinations/services and all interpretation and reporting for beneficiaries of the Department of Veterans Affairs, in accordance with all terms, conditions, provisions and requirements listed in the performance work statement (PSW) in the solicitation package. All contract physicians must be board certified and will be required to complete application and provide information to complete the credentialing and privileging process through the VA. Contract will be awarded to the responsible offeror whose offer provides the best value to the Government. This solicitation is unrestricted. All qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to submit their proposal. Offers, including options shall be evaluated for fair and reasonable pricing, technical capabilities to perform services, location for Veteran access from the VA Mercer CBOC, and satisfactory past performance record. Offerors must be registered in the Central Contractor Registration data base at www.ccr.gov. The Government anticipates award of a Firm Fixed Price commercial item contract. The term of the contract will be for a base year with four (4) one year options to extend at the Government's digression. The NAICS code is 622110 or 621512. The procurement will be conducted utilizing the commercial item procedures of FAR 12 in conjunction with FAR 15. Solicitation packages will be available on or about October 9, 2009 Proposals will be due on or about November 9, 2009. No hard copy solicitations or amendments will be provided. Solicitation documents may be obtained electronically at www.fedbizopps.gov. Potential offerors may obtain a copy of the solicitation VA-244-09-RP-0408 documents on-line at www.fedbizops.gov - Questions regarding this solicitation must be submitted in writing via email to the Contract Specialist, Roberta Smiley-Betzler, at roberta.smiley@va.gov and must include the solicitation number " VA-244-09-RP-0408 " and "RFI" on the subject line.
