- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation VA-244-09-RQ-0462 is issued as a request for quote (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. The procurement is being issued as a small-business set aside under NAICS code 339112, size standard 500 employees. The Wilkes Barre VAMC requires one (1) Optimum FCE (functional capacity evaluation) system: SYSTEM Interface AcuFORCE Dynamometer for Static Strength Testing, AcuGRIP Hand Grip Dynamometer, AcuPINCH Pinch Grip Dynamometer, AcuROM Spinal Inclinometer, and Heart Rate Monitors for Constant Heart Rate Monitoring FCESoft Software Suite MYOTEST Software for MMT, JobTask FCE Software, DOTSoft Software, Static FCE Software, and DTMSoft Software and DTM Tip for Low Back patients One Hand Static Handle and Large Disc for Upper Extremity Patients Omni 500 Static Lift Platform Lift Station Lifting Boxes Set Work Box 12x12 Open Top (straight & ergonomic handles), Overhead Lift and Carry Box 8x12 w/ Dowel Handle, Original Box 12x12x14 w/ Lid & Handles, and Real Box 12 Lift Box w/ Lid, but no Handles SLED for Dynamic Push-Pull Testing Purdue Pegboard for Fine Motor Skills Testing STEP Stacking CUBE Steel Disc Weights 20 each 10 lb discs, 5 each 5lb discs and 2 each 2.5 lb discs Weight Cart for SLED Accessory Package Reflex Hammer, Two Point Discrimination Gauge, C-128 Tuning Fork, Tally Counter, Stop Watch, Tape Measure, and 2 each PAIN-RPE Cards Color Notebook Computer and Color Inkjet Printer TBS FCE Certification 6 months Professional Customer Service and Technical Support 1 Year warranty 2 each service manuals and recalibration instructions The items called for by this RFQ has been identified by a brand name or equal description, such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of product that will be satisfactory. Equal products (including products of the brand name manufacturer other than the one described by brand name) will be considered for award if such products are clearly identified in the offer and are determined by the Government to meet fully the salient characteristics requirements listed in the RFQ: modular design for customization flexibility, complete systems that allows for full functional capacity evaluations; ability to create standardized or customized FCEs; over 160 validity criteria; testing protocols for dynamic infrequent, occasional and constant frequency; perform static whole body strength testing; spinal inclinometry, hand dynamometry and pinch grip dynamometry, and the ability to format & generate automatic reports. Shipping will be FOB Destination or Origin within 30 days ARO to Wilkes Barre VAMC, 1111 East End Blvd, Wilkes Barre, PA 18711.
