- SUPPLIES/SERVICES A.Services: Contractor is to provide Initial Evaluation and Rehabilitation Planning (Chapter 31) to the Vocational Rehabilitation and Employment Division, VA Regional Office, Salt Lake City, Utah in accordance with all terms, conditions, and requirements of this solicitation. 1.This contract is a basic ordering agreement in accordance with FAR 16.7 (see FAR clause 52.216-1, Type of Contract). A contract is formed, in accordance with the terms and conditions of this agreement, when the VA Regional Office agrees to place an eligible veteran pursuant to 38 U.S.C. 31 with the Contractor, and the Contractor agrees to accept the veteran. Upon acceptance by the Contractor, all terms and conditions of this agreement shall apply during such time as a VA patient remains under the care of the Contractor. 2.The contract will be for a base period of six (6) months with two (2) three-month options for renewal as identified herein. The contract commencement date is expected to be on or about October 30, 2009. Contractor shall be required to provide all services listed below, for which they are awarded a contract.
