- The Louis Stokes Cleveland Department of Veterans Affairs Medical Center, Cleveland, Ohio has a requirement for Medical Transportation (Ambulance) Service, to include Advance Life Support (ALS) and Basic Life Support (BLS). The contractor shall provide all labor, material, equipment, vehicles, vehicle maintenance, documentation, reports, medical personnel and supervision necessary to provide Ambulance service for beneficiaries of the Louis Stokes Cleveland VA medical Center and all CBOC locations. Contractor shall provide all Advanced Life Support (ALS) Ambulance Service and Basic Life Support (BLS) Ambulance Service, 24 hours per day, 365 days per year, in accordance with all terms, conditions, and schedule contained in the solicitation. Contractor shall furnish vehicles sufficient to provide full and complete coverage for pick-up and delivery of all veteran patients requiring transport by ambulance to/from the Louis Stokes Cleveland VA Medical Center, 10000 Brecksville Road, Brecksville, OH 44141, Louis Stokes Cleveland VA Medical Center 10701 East Blvd., Cleveland, OH 44106 and all CBOC locations to include: Akron CBOC, 55 West Waterloo Road, Akron, OH 44319; Akron CBOC, 95 West Waterloo Road, Akron, OH 44310; Canton CBOC, 733 Market Avenue, Canton, OH 44702; East Liverpool/Calcutta CBOC, 15655 State Route 170, Calcutta, OH 43920; Lorain CBOC, 205 West 20th Street, Lorain, OH 44052; Mansfield CBOC, 1456 Park Avenue West, Mansfield, OH 44906; McCafferty CBOC, 4242 Lorain Avenue, Cleveland, OH 44113; New Philadelphia CBOC, 1260 Monroe Street NW, Suite #1A, New Philadelphia, OH 44663; Painesville CBOC, 7 West Jackson Street, Painesville, OH 44077; Painesville CBOC, 54 South State Street, Second Floor, Suite 204, Painesville, OH 44077; Ravenna CBOC, 6751 North Chestnut Street, Ravenna, OH 44266; Sandusky CBOC, 3416 Columbus Avenue, Sandusky, OH 44870; Warren CBOC, Riverside Square, 1400 Tod Avenue, Warren, OH 44485; Wooster CBOC, 124 North Walnut Street, Wooster, OH 44691; and Youngstown CBOC, 2031 Belmont Avenue, Youngstown, OH 44505. The VA Contracting Officer reserves the right to issue multiple awards as a result of this solicitation. Multiple awards shall be made based upon geographic zone. The solicitation will be in accordance with FAR Part 12 Acquisition of Commercial Items, and shall be 100% set-aside for small business concerns in accordance with FAR 6.203. The North American Industrial Classification System (NAICS) code for this requirement is 621910; size standard $7.0 million per annum. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The Contract period is for base year with four year options. The solicitation shall be posted to the FedBizOpps website on or about October 30, 2009 as a request for proposal. Responses to the solicitation shall be due by close of business on or about December 15, 2009. Offerors are advised that it is your responsibility to review and monitor the website frequently to ensure you have the most up to date information, including amendments.
