Description
The VA Medical Center, VAMC, 1 VA Center, Augusta, Maine has a requirement for Adult Day Care Health Services in the Sanford, Maine area. ADHC will be furnished to veterans for whom such care is specifically authorized by the Department of Veterans Affairs. The purposes of providing ADHC are: to enable functionally impaired individuals to reside in supportive home environments rather than in nursing home care facilities; to facilitate the return of older veterans to their home and coordinate long term care thereby reducing readmission's to hospitals and nursing homes; to maintain the older veteran at the highest level of functioning possible; to improve the quality of life for the participants among their peers; and to provide support for the family and other care givers of older veterans. The care provided will include: nursing, social work services, assistance with activities of daily living, activity therapy, case management, nutrition service including meals and transportation. The ADHC Program will meet the VA standards for ADHS as promulgated in 38 CFR Part 17.51c through 17.51e. Multiple awards may be made in order to service the entire Sanford Maine area. See additional requirements in the solicitation. The applicable NAICS Code is 624120 with a size standard of $7.0 Million and Estimated Price Range is between $25,000 and $100,000. This acquisition is a full and open competition, Healthcare Resources Basic Ordering Agreement (HCR-BOA).
