Description
The VA Montana Healthcare System is seeking a Certified Diabetes Educator (CDE) to provide certified diabetes education and nutrition education to veterans in the Kalispell, Mt area.The contractor will provide Diabetes Self Management Education (DSME) for veterans referred by consult according to the National Standards for DSME. Classes will be established by contractor in a conference room setting for group classes and an office setting for one on one education that will ensure patient privacy is honored under the patient privacy regulations. A two hour group class that can accommodate 10 patients at a time will be offered monthly designed for newly diagnosed diabetics to teach the basics of monitoring and nutrition. For the complete diabetes education program, twelve 8 hour classes (1 per month) will be scheduled that can accommodate 10 patients at a time. The class meets for 2 hours weekly for four weeks in a row. Veterans that have special needs that would make class participation unacceptable would meet with the educator one on one. One on one education sessions will be reserved for only those veteran requiring individual education sessions. The Certified Diabetes Educator (CDE) conducting the education will work with the physician ordering the consult to make the determination for the veteran to be seen individually. The contractor will provide appropriate printed educational materials for veterans home use that will compliment the education received in the sessions with the educators. This acquisition is open to both small and large business concerns. The NIACS code for this acquisition is 611710 with a size standard of $7.0 million. Solicitation number VA259-10-RP-0010 has been assigned and should be referenced in all correspondence regarding this announcement and this solicitation. NO HARD COPIES OF THE SOLICITATION WILL BE ISSUED.
