- DEPARTMENT OF VETERANS AFFAIRSVA Medical Center CSCMichigan 5500 Armstrong Road Battle Creek, MI 49037 Date: In Reply Refer to: 515/90VC CM Governmental Services, LLC 310 Custer Drive Battle Creek, MI 49037 Subject: Notice of Award, Contract VA251-C-0665, Recovery Act: Replace Roads Curbs and Gutters Phase V Dear Mr. Freidersdorf: Acceptance is hereby made of your offer dated 24 Jun 2009 which was submitted in response to Invitation for Bid VA-251-09-RA-0168, Entire Project, to furnish all services, labor, materials and equipment for Recovery Act: Replace Roads Curbs and Gutters Phase V at the Department of Veterans Affairs Medical Center, Battle Creek, Michigan. Award is for consideration of Nine Hundred Ninety NineThousand Fifty Dollars and No Cents ($999,050.00), in strict accordance with the contract terms and project specifications. A fully executed copy of the contract is sent herewith. The contract allows One Hundred Thirty Five (135) calendar days for completion of the project after issuance of the Notice to Proceed. You are required to furnish 100% payment and performance bonds in a penal sum equivalent to the contract award amount. The bond forms are enclosed for your convenience. In accordance with your accepted offer, the bonds must be executed and returned within ten (10) calendar days after receipt of this award letter. The bonding company used must be acceptable in accordance with the Department of Treasury Surety Listing TD 570. A checklist to insure acceptable bonds are submitted is also enclosed for the surety's use. Please use the checklist to insure the bonds are prepared accurately because if found defective during review the bonds will be rejected and returned for resubmission. A preconstruction meeting will be scheduled after acceptable bonds have been provided. A Notice to Proceed will be issued at this time which is necessary before any work begins or materials are ordered. Please acknowledge receipt of this letter.
