J--PREVENTIVE MAINTENANCE (PM) AND REPAIR FOR ANESTHESIA EQUIPMENT (3 EA)- DRAGER FABIUS GS SERIAL NUMBERS (SN) ARWK-0140, ARWK-0141, ARWK-0142; (4 EA)- D-VAPOR SERIAL NUMBERS ARWB-0148, ARWB-0256, ARWE-0249, ARWE-0497 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number VA-245-10-RQ-0013 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. The procurement is full and open competition; all responsible sources may submit a quotation, under NAICS code 811219 and the business size standard is $7.0. The Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for performance under this contract beyond 9/30/, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer; FAR 52.217-8 Option to Extend Services (Nov 1999)- The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days; FAR 52.217-9 Option to Extend the Term of the Contract (Mar 2000)- (a) The Government may extend the term of this contract by written notice to the Contractor within 30 days provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years and 6 months; FAR 52.228-5 Insurance-Work on Government Installation (Jan 1997) to include the following addenda: 28.307-2 Supplemental Insurance Information Liability- (a) Workers' compensation and employer's liability. Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.) (b) General liability. (1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence (2) Property damage liability insurance shall be required only in special circumstances as determined by the agency (c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims; VAAR 852.203-70 Commercial Advertising (Jan 2008); VAAR 852.233-70 Protest Content/Alternative Dispute Resolution; VAAR 852.270-1 Representatives of Contracting Officers; receipt of equipment, supplies, and nonpersonal services (Jan 2008); VAAR 852.273-75 Security Requirements For Unclassified Information Technology Resources (Interim Oct 2008).
