- PAST PERFORMANCE LINE ITEMS STATEMENT OF WORK HSBP1010R2854 HSBP1010R2854 (COVER) The Customs and Border Protection (CBP) Office at JFK International Airport, New York has a recurring requirement to obtain medical services for suspects with internally concealed contraband, including but not limited to, heroin, cocaine, marijuana, hashish, illegal pills, and currency. The Request for Proposal (RFP) number is HSBP1009R2854. The Contractor shall provide all required facilities, supplies, equipment, management, professional personnel and staff to provide the medical services specified herein. These services will support the CBP mission of the detection and interception of illegal drugs. The contractor’s facility must be a comprehensive occupation health facility offering on-site medical, including physician/nurse services, twenty-four hours daily. The location of the Medical facility shall be within the confines of JFK Airport grounds as considered by the Port Authority of NY/NJ. The Contractor will be responsible for fulfilling these location requirements. The Medical facility must be located east of the Howard Beach JFK Airport train station (long term parking facility) and south of the Nassau Expressway on the western portion of JFK and south of North Boundary road/Rockaway Blvd on the eastern portion of JFK. In addition the Medical facility shall be in close proximity of a Port Authority field access post (within ¼ of a mile). The Contractor shall provide adequate parking for CBP Officers when assigned to the Medical facility on official business. The solicitation document, incorporated provisions and clauses are those in effect Federal Acquisition Regulation FAC 2005-37. The North American Industrial Classification System Code is: 621112 This requirement is full and open. Offerors may submit proposals via email to: brice.winston@dhs.gov and must be submitted by 06 November 12:00pm, Eastern Standard Time. Questions/Offers submitted by a method other than electronic mail shall not be accepted and shall not be answered. Questions with the government’s responses shall be posted to FBO. The U.S. Customs and Border Protection (CBP) requires the following: *** PLEASE REFER TO THE ATTACHED STATEMENT OF WORK (SOW) FOR SCOPE OF WORK AND REQUIREMENTS. AWARD EVALUATION CRITERIA: CBP will evaluate your firm (in descending order of importance) using the evaluation criteria described as follows (Ref: Section M): I. Technical/Management Approach: Provide office space dimensions and demonstrate your firm’s ability to accommodate CBP Government Furnished Property (Drugloo Type 1025) and provide a secure 24-hour facility capability as required by the Statement of Work. Describe your firm’s staffing plan, including the experience and expertise of your staff with government agencies or similar support. Describe your firm’s technical approach in meeting the requirements contained in the SOW. Include a discussion of the company’s demonstrated ability and capability to effectively and efficiently perform the requirements. II. Past Performance (Ref: Section J): Provide three references for like or similar work done for federal and/or non-federal government agencies within the last three (3) years. Please ensure that each reference includes complete contact information. Please include either the Project Manager as Point of Contact for each reference. Demonstrate experience in a contract of similar size and complexity to that contained in the Statement of Work. This synopsis will be used to assess the experience and capability in doing the work specified in the SOW. In case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror shall not be evaluated favorably or unfavorably. III. Price: The price proposal will be evaluated separately for price reasonableness. The proposal of all offerors will be evaluated in order to determine whether they are reasonable in relation to the work to be performed, reflect a clear understanding of the requirements and reflect a clear understanding of the technical proposal Price may become increasingly more import as differences in technical rating decreases. If the technical features of the responses are determined to be essentially equivalent, the cost/price may become the determining factor for award. The determination that technical solutions are technically equivalent is at the discretion of the source selection official. Proposal must include the solicitation number along with your Tax Identification Number (TIN), DUN and Bradstreet number (DUNS), complete Business Mailing Address, contact name, contact phone number, contact fax number, and contact email address.
