- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-37 as corrected. This solicitation is unrestricted. The NAICS code is 621511, with a size standard of $13.5 million. All procedures (excluding pre-biopsy) shall be performed at the contractor's facility. The contract will include a one year basic period and two one-year options. Only electronic offers will be accepted. Offers are due to megan.barr@va.gov by NLT 3:00 p.m. on November 24, 2009. The Statement of Work and Price Schedule are as follows: Scope of Work: The Contractor shall provide personnel, equipment, materials, and transportation of specimens to perform diagnostic liver biopsy for assessment of chronic Hepatitis C infection and report the biopsy findings to the VASNHCS. All procedures (excluding pre-biopsy) shall be performed at the contractor's facility. 2. Specific Contractor tasks: 2.1. Pre-Biopsy requirements are performed by the VA at VASNHCS as follows: A history and physical from the referring doctor or facility will be provided, current lab work with PTT, HCT, and HGB will be provided, and no blood thinners (Coumadin, aspirin or heparin) will be used for five (5) days prior to procedure. 2.2. Day of Exam: The patient will be required to arrive 30 minutes prior to the exam, nothing by mouth 4 hours prior to exam, and the patient will stay two (2) to four (4) hours post-exam. 2.3. Post-procedure: The patient will be monitored by an RN, radiologist or other support staff for two (2) to four (4) hours following the exam, the patient will be resting on a gurney with BP, pulse and respiration monitored, the patient will be discharged with radiologist approval. If there is a complication following the procedure, once the patient is relatively stable, the contractor will call the VASNHCS triage at 775-328-1214 and discuss appropriate transfer of the patient back to the VA. If the patient is unstable post-procedure the contractor retains the option of calling the nearest emergency room and arrange for appropriate transfer. The contractor will notify the VASNHCS immediately. Contractor will call VASNHCS, Medical Service, at 775-328-1747 and/or 775-328-1427 to alert them that a specimen is ready for pick-up. 2.4. Report: The contractor shall prepare a biopsy report and fax it to VASNHCS to 775-328-1769 within 24 hours of the procedure. 2.5. Schedule patients within 30 days of notification. 2.6. Transportation: VASNHCS will arrange for specimen pick up. Contractor must notify VASNHCS, Medical Service, when a specimen is ready for pick-up. 2.7. Invoicing: Payment will be based on the rates listed in the Price Schedule for the following CPT codes: Needle (percutaneous) liver biopsy (CPT Code 47000) CT Fluoroscopic Guidance (CPT Code 76003), Computer Tomography (CT Guidance (CPT Code 76360), Magnetic Resonance Imaging (MRI) Guidance (CPT Code 76393), Ultrasonic Guidance (CPT Code 76942), Conscious Sedation (CPT Code 99144).
