- *******RECOVERY ACT******VA Sierra Pacific Network (VISN 21) is issuing a solicitation limited to Service Disabled Veteran Owned Small Business (SDVOSB) companies pursuant to the authority set forth in 38 USC 8127. This procurement is a total 100% SDVOSB set-aside. A contract will be awarded to the selected SDVOSB Contractor to perform the following requirements: Community Living Center (CLC) Homelike Environment Renovation Scope of Work CLC Homelike Renovation, Phase 2 Project # 654-09-401 October 15, 2009 1)PURPOSE: a)The Contractor shall provide all labor, material and equipment necessary to accomplish construction for renovations of the west core area of the Community Living Center (CLC), Building 10, at the VA Sierra Nevada Health Care System (VASNHCS) facility in Reno, Nevada as necessary to provide a more homelike environment within the CLC. The total area to be renovated under this scope of work is approximately 1,500 square feet. Work includes general construction, alterations, mechanical and electrical work, utility systems, nurse call systems, necessary removal of existing structures and construction and certain other items. 2)SCOPE: a)The following is provided to clarify the scope of this Project: b)Location of Project is 1000 Locust St Reno Nevada 89502. Affected area is located on the 1st floor Building 10. c)Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for tenant improvement in nursing and support areas as required by drawings and specifications. d)Timeline is six calendar months from issue of notice to proceed. e)The ultimate goal for this project is to have an upgraded interior finish that resembles a homelike environment while maintaining the required standards of care for the resident veteran population; these areas are to include but not limited to Caregiver Stations, Dining Areas, Common Living Areas, Resident Laundry, Clean and Soiled Utility Rooms, Medication Rooms. 3)CONSTRUCTION CONSIDERATION a)All work will be required to meet Infection Control Class IV. b)Meet all conditions on current VA ICRA Permit for "Work During Project" and "At Completion of Project". c)Problems shall be addressed immediately to the COTR; Contracting Officer is the only person to authorize changes or modifications of the project. d)In general, all work shall be accomplished during a standard tour between 8 AM and 4:30 PM, Monday through Friday. However, some work may not be available during the listed hours. For these areas the contractor shall accomplish work during "off tour" times. These areas include but are not limited to, electrical or utility shutdowns that affect areas beyond the construction boundaries. This work will be defined and agreed upon prior to the start of any construction activities. e)All Contractors will need to comply with the DVA Personal Identification Verification (PIV) Requirements as well as Flu and TB testing. i)Contractors to provide proof of negative TB test via shot record or doctors note to contracting officer. ii)Contractors shall indicate if they have had a seasonal flu shot for the current flu season. Verbal acknowledgment of flu shot is acceptable. iii)Contractors can submit a cumulative list by employee name for all employees and sub contractors prior to the employee or sub contractor commencing work at the facility. The NAICS Code for this construction project is 236220 with Small Business Size Standard of $33.5 million. The magnitude of construction for this requirement is between $500,000 and $1,000,000.
