Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, and as supplemented with additional information included in this notice. See attached Statement of Work (SOW) and Evaluation Criteria for detailed description of requirement. This announcement is the only solicitation; quotations are being requested and a written solicitation will not be issued. The reference number is VA-249-10-RQ-0043 and is issued as a Request for quote (RFQ). Offerors must be registered in Contractor Central Registration (CCR). This acquisition is firm-fixed-price. A firm requirement of this solicitation is that the contractor have access to a cyclotron and lab within a 60-mile radius of the VA Medical Center at Nashville TN to produce this FDG Isotope. Offerors may indicate that their response to FAR 52.212-3 Offeror Representations and Certifications-Commercial Items has been loaded on http://orca.bpn.gov. Submit one (1) electronic copy of price quote and capability statement to Sandra.magers@va.gov. Responses shall be submitted no later than 2:00 pm (Central Standard Time) 03 December 2009. Quotes must be valid for thirty (30) days after due date. Any amendment(s) issued to this RFQ will be published on this website. Therefore, it is the offeror's responsibility to visit this website frequently for updates on this procurement.STATEMENT OF WORK AND EVALUATION CRITERIA Radiopharmaceutical unit doses for Nuclear Medicine - PET Scanning FDG A.DELIVERY SERVICE: Quantities expressed in the schedule are estimates based on estimated usage for one year. However, quantities may vary based on patient demand. 1.Contractor shall deliver the unit doses of 18F-radiolabeled fluoro-de-oxy-D-glucose (hereafter referred to as FDG) directly to the Nashville Nuclear Medicine Department of Veterans Affairs Medical Center, 1310 24th Ave. South., Nashville TN, PET/CT suite, to the "Hot Lab." 2.Contractor must have access to local cyclotron and lab, not to exceed 60 miles distance from Nashville Nuclear Medicine Department of Veterans Affairs Medical Center, 1310 24th Ave., South, Nashville TN 37212. 3.Delivery of all items shall be provided in unit dose packaging. Items shall be required Monday through Friday during the normal duty hours of 6:30 a.m. and 4:30 p.m. on an "as needed" basis. The first "run" of doses shall be delivered to the "Hot Lab" at the Nashville VA Medical Center on or before 6:30am M-F. Delivery to the Nashville VA Medical Center shall be made within 40 minutes after order is placed, unless delivered in a pre-scheduled, twice-a-day delivery, with the unit dose "pre-decayed" for the prescribed activity to be present in the dosing syringe at the time of the scheduled examination. Non-scheduled doses (add-on and emergency patient doses) must be delivered, also pre-decayed for the time of scheduled injection, within 40 minutes of the placement of the order, if the order is placed within normal working hours. Estimated average unit dose will be 15mCi. 4.A dose may be cancelled free of charge by the VA if the contractor is notified at least 90 minutes prior to calibration of the specific dose and/or delivery of scheduled doses. 5.The Contractor will provide the requested FDG unit doses during all normal working days of the Tennessee Valley VA Healthcare System. Weekends and Federal holidays are not considered normal working days. 6.All FDG unit doses will be certified by the Contractor as ready to inject into human patients, with the Contractor being responsible for all quality control, sterility and pH-testing, etc., that is part of the normal practice of radiopharmacy in accordance with all applicable FDA and NRC regulations. The Contractor is responsible for all transportation of the unit doses, including radiation shielding, and will accept used unit dose syringes back as waste to be disposed of at the Contractor's expense as often as needed by the Nashville TVH VA Medical Center. 7.Contractor will provide consultation service regarding radiopharmaceutical applications, problems, controls, etc., upon request at no charge. Doses that are needed for the routine calibration or servicing of the TVHS VA PET/CT scanner in accordance with the manufacturer's recommendations will be provided by the Contractor free of charge, not to exceed 8/year. Estimated range of activity will be 500uci-25mCi. 8.Any doses deemed below standard ( e.g., insufficient activity and/or volume, spilled or partially-spilled doses) by the TVHS VA Medical Center Nuclear Medicine Department personnel shall be credited toward the account, and any replacement dose(s) shall be charged to the VA at a 15% reduction in cost for that dose. B.DELIVERABLES AND DELIVERY SCHEDULE First "run" of the Isotope is to be delivered to the VAMC Nashville "Hot Lab" before 6:30am. C.PERIOD OF PERFORMANCE The period of performance is to be 12/21/09 though 12/31/10, with four 12-month options to renew the contract at the discretion of the VAMC Nashville. D.EVALUATION FACTORS FOR AWARD Award will be based on the following evaluation factors. Technical capability, proximity of cyclotron and hot lab to VAMC Nashville, TN, and past performance will be rated in descending order of importance. All three technical factors are significantly more important than price. 1.Technical experience in producing FDG Isotope 2.Production proximity to VAMC Nashville, TN. The site of the cyclotron and hot lab to be used to produce the FDG Isotope must be located within a 60-mile radius of the VAMC Nashville. 3.Past Performance. The offeror shall provide three references to include name, name of agency or firm for which work was performed, and phone number. The extent to which the offeror provides evidence of past performance as measured by the following factors to include work similar in size and scope for the work required under this solicitation performed within the past three years. a)Quality of products and services as well as technical support provided, to include quality of personnel; b)Cost control; c)Timeliness of delivery; d)Business relations, to include problem responsiveness; and, e)Customer satisfaction.
