Description
The Louis Stokes Cleveland VA Medical Center is interested in identifying sources to provide Dialysis Services. This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, hubzone, 8(a), small disadvantage business, small business, or large business) relative to NACIS 621492 (size standard of 13.5M per annum). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, a service capability statement, three customer references to which you have provided similar services to, copies of applicable licensing, address, point of contact, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified hubzone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business? Interested Contractors shall provide all of the following equipment, supplies, and services when rendering chronic hemodialysis at the contractor-owned facility. Items covered currently in the Medicare composite rate Routine administration of medications used during the dialysis runs Dialysis related medical supplies, staff labor, equipment and facility use Care planning, dietary and social services counseling Erythropoietin (Epogen, Procrit, Aranesp) used during the dialysis runs Injectable Vitamin D analogs (Calcijex, Hectoral, Zemplar) Intravenous iron products (Infed, Dexferrum, Ferrlecit, Venofer) Routine dialysates, artificial dialyzers, bloodiness, and other supplies Routine dressing changes and suture removal Routine pharmaceutical items included in the Medicare composite rate such as albumin, Benadryl, dextrose, glucose, heparin, mannitol, plasminate, normal saline and sodium chloride categories Use oxygen as indicated EKG's as ordered and interpretation Intravenous (IV) antibiotics Hepatitis B vaccinations Laboratory testing specifically ordered by patients nephrologists Declotting agents used to treat ESRD shunts, peripheral lines or arteriovenous (AV) fistulas The Contractor shall have a network of providers that is sufficient to provide chronic hemodialysis outpatient care to VA-eligible patients within these geographical areas of Ohio: Akron Brecksville Calcutta Canton Cleveland Lorain Mansfield New Philadelphia Painesville Ravenna Sandusky Warren Wooster Youngstown Responses to this notice shall be e-mailed. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Response to this Sources Sought is not a request to be added to a prospective bidders list or to receive a copy of future solicitations.
