- DEPARTMENT OF VETERANS AFFAIRS (VA)PALO ALTO HEALTH CARE SYSTEM (PAHCS) MENLO PARK DIVISION (MPD) LOOP ROAD COMBINED PROJECT MENLO PARK, SAN MATEO COUNTY, CA SCOPE OF WORK These three American Recovery and Reinvestment Act (ARRA) projects are combined for project management, cost effectiveness, and other administrative considerations. All three are within scope of each other, related to each other, and next to each other geographically. They were originally one project; however, sufficient funds were not available to create one project, prior to passage of American Recovery and Reinvestment Act (ARRA). With ARRA, funding issue was resolved. There is no bundling as defined in FAR 2.101. Summary Project Title: Loop Road. Recovery Act (RA) Solicitation (RFP) Number: VA-261-10-RA-0033 Project Numbers and Titles combined: 640A0-09-150M FCA, Loop Road, Phase 1, New Roadways 640A0-09-243M FCA, Loop Road, Phase 1, New Parking Lots 640A0-09-263M, Loop Road, Phase 3, New Roadways and New Parking Lots Project Location: VA, PAHCS, Menlo Park, San Mateo County, CA Project Estimated Price Range: $5,000,000 and $10,000,000 Acquisition shall be conducted in accordance with FAR 15.101-2 - Lowest Price Technically Acceptable Source Selection Process. (a) The lowest price technically acceptable source selection process is appropriate when best value is expected to result from selection of the technically acceptable proposal with the lowest evaluated price. RFP to be issued electronically at FBO Website RFP shall be issued on or around December 04, 2009 Offer due date shall be on or around January 08, 2010 Contracting Activity: Department of Veterans Affairs (VA) VA Sierra Pacific Network (VISN 21) Consolidated Contracting Activity (CCA) Bldg 209, Room 126 5342 Dudley Blvd McClellan Park, Sacramento County, CA 95652 Engineering Activity: Department of Veterans Affairs (VA) Palo Alto Health Care System (PAHCS) 3801 Miranda Avenue Palo Alto, Santo Clara County, CA 94304 1.2 STATEMENT OF BID ITEM(S) A. SCOPE OF WORK PROJECT 640A0-09-150M FCA "Loop Road, Phase 1" Three projects are combined for project management, cost effectiveness, and other administrative considerations. The contractor shall provide all materials, labor, tools, equipment, transportation, expertise, supervision, and administration to construct approximately 970 linear feet of new roadway located at the Menlo Park Division (MPD) of the Department Veterans Affairs (VA) Palo Alto Health Care System (PAHCS). Work includes the following: Demolition of fences, asphalt paving, concrete walkways and catch basins, etc. Demolition of residential garages of quarters 305 and 306. The garages have no known asbestos containing materials. The garages are assumed to have leaded based paint. Contractor shall submit for approval a plan for dust and paint chip control. Disposal legally of demolished debris. Removal of trees. Clearing and grubbing. General grading and compaction. Install storm drainage system complete w/piping, manholes, curb inlets, catch and settling basins, etc. Relocate fire hydrants. Amend soil. Adjust existing manholes, meter and valves to grade. Install 14.5" class II AB gravel. Install 4" asphalt concrete, graded and compacted. Apply general thermoplastic stripping. Install new concrete sidewalks. Install truncated domes for ADA. Install lighting system complete with conduits, conductors, pedestrian light path, double & single arm street light pole, concrete pedestal, etc. Install new landscaping complete with irrigation system, river rock rip rap, ground covers, shrubs, trees, sods, mulch, etc. Install erosion control. Install CMU trash enclosure fencing. Paint curbs. The contractor's work schedule shall include detail closure and interim traffic circulation plan coordinated with the COTR and VA Police. Some earthmoving and paving work may need to be done during weekends or after hours. All work shall be in accordance with the contract drawings and specifications for Project 640A0-09-150M FCA, Loop Road, Phase 1. 1.2 STATEMENT OF BID ITEM(S) B. SCOPE OF WORK PROJECT 640A0-09-243M FCA "Loop Road, Phase 1 - Parking Lots" Three projects are combined for project management, cost effectiveness, and other administrative considerations. The contractor shall provide all materials, labor, tools, equipment, transportation, expertise, supervision, and administration to construct approximately 78,000 square feet of parking lots located at the Menlo Park Division (MPD) of the Department Veterans Affairs (VA) Palo Alto Health Care System (PAHCS). Work includes the following: Demolition of fences, asphalt pavement, concrete sidewalks, catch basins, etc. Demolition of residence and garage of Quarter 60. The quarter roof and kitchen areas were abated for asbestos. Any additional areas tested for asbestos by the contractor must be abated prior to demolition. The quarter is assumed to have lead paint. Contractor shall submit a plan for approval for dust and chip control. Legally disposal of demolished debris. Removal of various trees as shown on the tree removal plan. Clearing and grubbing. General grading and compaction. Install storm drainage system complete w/piping, manholes, curb inlets, catch and settling basins, etc. Relocate fire hydrants. Amend soil. Adjust existing manholes, meter and valves to grade. Install 14.5 class II AB gravel. Install asphalt concrete, graded and compacted. Apply general thermoplastic stripping. Install new concrete sidewalks. Install truncated domes for ADA. Install lighting system complete with conduits, conductors, pedestrian light path, double & single arm street light poles, concrete pedestal, etc. Install new landscaping complete with irrigation system, river rock rip rap, ground covers, shrubs, trees, sods, mulch, etc. Install erosion control. Apply traffic stripping and parking lot stripping. Paint curbs. The contractor's work schedule shall include detail closure and interim traffic circulation plan coordinated with the COTR and V.A. Police. Some earthmoving and paving work may need to be done during weekends or after hours.All work shall be in accordance with the contract drawings and specifications for Project 640A0-09-243M FCA, Loop Road, Phase 1, Parking Lots. 1.2 STATEMENT OF BID ITEM(S) C. SCOPE OF WORK PROJECT 640A0-09-263M "Loop Road, Phase 3" Three projects are combined for project management, cost effectiveness, and other administrative considerations. The contractor shall provide all materials, labor, tools, equipment, transportation, expertise, supervision, and administration to construct approximately 2,100 Linear Feet of new roadway and 28,800 Square Feet of parking lots located at the Menlo Park Division (MPD) of the Department of Veterans Affairs (VA) Palo Alto Health Care System (PAHCS). Works are as follows: Demolition of asphalt paving, concrete walkways and catch basins, etc. Disposal legally of demolished debris Removal of trees Clearing and grubbing General grading and compaction Install storm drainage system complete w/ piping, manholes, curb inlets, catch and settling basins etc. Relocate fire hydrants Amend soil Adjust existing manholes, meter and valves to grade Install 14.5" thick class II AB gravel Install 3-1/2" asphalt concrete, graded and compacted Apply general thermoplastic stripping Install new concrete sidewalks and walkways Install truncated domes for ADA Install lighting system complete with conduits, conductors, pedestrian light path, double & single arm street light pole, concrete pedestal, etc. Install new landscaping complete with irrigation system, river rock rip rap, ground covers, shrubs, trees, sods, mulch, etc. Install erosion control Paint curbs The contractor's work schedule shall include detail closure and interim traffic circulation plan coordinated with the COTR and V.A.
