SOW Attachment #3: Enlarged "As Built" Drawing (1995) with Gov.-added color-coded Zones and markings to show current areas requiring contractor upgrade/work. SOW Attachment #2; Hytrol parts information #2; [2 pages] SOW Attachment #1; Hytrol parts information; [2 pages] STATEMENT OF WORK (23 November 2009); Upgrade of Existing, Powered Interlake Roller Conveyor System at 96 Logistics Readiness Squadron, Cargo Movement Element, Building 613, Eglin AFB, FL [22 pages] The 96 Contracting Squadron, Eglin AFB, FL, intends to solicit, negotiate, and award a Firm-Fixed Price contract for purchase of Overhaul/Upgrade of Receiving Storage Distribution System (RSDS) [cargo conveyor system] manufactured by International Automated Systems, Inc., in accordance with the attached Statement of Work (SOW). This requirement is for an Upgrade to the Existing System, and NOT for a Replacement System. This acquisition is under Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and Part 13.5, Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; a quotation is being requested, and a written request for quotation will not be issued. This solicitation is issued as Request for Quotation (RFQ) F1T3BU9294A001. The North American Industry Classification System (NAICS) for this acquisition is 333922, small business size standard: 500. Please identify your business size in your response based upon this standard. This solicitation is 100% small business set-aside. Proposals are requested with CLINs broken down by labor (including proposed labor categories), materials and any travel required in accordance with the Joint Travel Regulation. Contractor is responsible for removal and disposal of the replaced existing equipment; however, Gov. customer 96LRS/LGRDDC will be uninstalling the current conveyor system. Some sections of existing systems are not accessible by forklift and should be considered in removal/disposal plan. Delivery is FOB Destination/jobsite and all Equipment/Materials will be delivered to, and work will be performed at, 96 LRS/LGRDDC, 201 Biscayne Rd, Eglin AFB, FL 32542. Contractor will provide manufacturer's warranty information for all equipment upgraded to 96 LRS/LGRDDC. EVALUATION OF OFFERS: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. We are using the lowest price technical acceptable method (LPTA) and award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. The following factors shall be used to evaluate offers: (1) TECHNICAL ACCEPTABILITY (2) PRICE Technical and Price, when combined, are EQUAL. However, proposals which are not rated as Technically Acceptable will be eliminated from the competition and receive no further consideration. 1. All proposals timely received will be evaluated and an award will be made to the lowest priced, technically acceptable, responsible offeror who responds to all items included in the solicitation (all or none) and complies with the terms of the Request for Proposal (RFP) and the SOW. This requirement is for an Upgrade to the Existing System, and NOT for a Replacement System. Any offers involving replacement systems will be found Technically Unacceptable. 2. These are the technical areas that we will evaluate to determine the technical acceptability: (a) Quality of Product - Offers will be evaluated on the proposed new equipment features and their compatibility to the existing system. This includes but is not limited to: controls, power supplies, motors, rollers, sensors, instrumentation and computer upgrade. Specifications for all equipment (if other than those suggested in the SOW) should accompany the Price Proposal. (b) Enhancement of System - Offers will be evaluated for suggested system or zone enhancements to make upgraded system more efficient and easier to maintain. Please explain suggested system enhancements and the cost impacts (price vs long term savings). This requirement is for an Upgrade to the Existing System, and NOT for a Replacement System. Any offers involving replacement systems will be found Technically Unacceptable. Schedule - Offerors should propose an overall schedule and associated downtime that customer will have. A site visit is scheduled for 1330 (local time is CST) on 8 Dec 2009 at: 96 LRS/LGRDDC, 201 Biscayne Rd, Eglin AFB, FL 32542. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter is required regardless of the length of visit and will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the contract specialist for processing into the Security Forces data base. Contractors requiring base access for meetings or site visits must complete the Eglin CAB (Contractor Access Badge) Affidavit. Contractors requiring access to work on base will need to complete the Eglin CAB Affidavit and a Form I-9 (Employment Eligibility Verification). These documents must be delivered to the Eglin AFB East Gate Pass and ID Office in a sealed envelope with a point of contact (POC) and phone number on the front of the envelope. You may also mail or FEDX the documents to: 96 Security Forces Squadron, Bldg 272 302 West D Ave Eglin AFB FL 32542 Attn Pass & Registration Anticipate a three-day lead time for Security Forces to process the documents and conduct the required background check. Once the background check is complete, a copy will be returned to the POC listed on the envelope. The prime contractor can provide paperwork for their subcontractor(s) and submit as many documents per envelope as necessary. (c) When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate and social security card to obtain a vehicle pass. All deliveries to contractor work sites on Eglin AFB main base must go through the North Gate, located off SR-85 South. Trucks trying to enter the East, West or the ACC gate will be turned around and told to use the North Gate. Contractors' work trucks carrying equipment and tools are also required to use the North Gate. (d) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (e) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management citing the appropriate paragraphs as applicable. (f) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (g) Failure to comply with these requirements may result in withholding of final payment. (End of clause) 5352.223-9001 - HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (JUN 1997) (a) In performing work under this contract on a Government installation, the contractor shall: (1) Comply with the specific health and safety requirements established by this contract; (2) Comply with the health and safety rules of the Government installation that concern related activities not directly addressed in this contract; (3) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (4) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (c) The contracting officer may, by written order, direct Air Force Occupational Safety and Health Standards (AFOSH) and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (d) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.
