- THIS IS A RECOVERY ACT PROJECT This is a request for Standard Form 330, not a request for proposal. Provide Professional Architectural Engineering Design services for the Relocation of Psychology Department at Jesse Brown VA Medical Center, Chicago, Illinois. This work includes providing professional architectural and engineering services for JB 537-08-114 Relocation of Psychology Department at Jesse Brown VAMC, 820 South Damen Avenue, Chicago IL. 60601. Shall provide professional services necessary for the design of this project at Jesse Brown VA Medical Center. This is primarily Architectural and Planning in scope and may have related Mechanical/Electrical/Plumbing/Fire Protection Engineering. Scope of work may include thorough Field Verification of existing conditions, preparation of schematic designs, construction documents, specifications, cost estimates, review of shop drawings & sample submittals, site visits and preparation of punch lists etc. for final acceptance. Scope of work includes remodeling approx. 50,000 SF on three floors (22000 on 8th, 15000 on 9th and 12500 on 10th) of existing space and shall include reception, waiting area, offices, meeting rooms, group rooms, conference rooms, treatment rooms, day hospital rooms, toilet areas and related support spaces to provide a functional, efficient, patient friendly clinic. Design includes providing easy access to all areas by disabled patients and staff. Scope of work shall include design of new HVAC system and distribution system, lighting and power design, plumbing and fire protection systems, GEMS requirements for specification of new materials and disposal of removed materials. Preliminary concept design have been developed for these areas and shall be used as a starting point to finalize Schematic designs. Contract documents will include division of the entire area in several independent phases of construction to provide ease of construction and to meet levels of available construction funds. Abatement of hazardous materials shall be contracted by VA directly with abatement contractor. Furniture and Owner Equipment shall be specified and purchased by VA but coordinated by AE. Area of consideration is restricted to 100 miles of Jesse Brown VA Medical Center, Chicago, Illinois. 2009. Selection criteria will be based on: (1) Past performance on similar projects, maintaining project schedules and project budgets; (2) Overall engineering design experience and capability with architectural, mechanical and electrical renovation projects; (3) Qualifications of staff to be assigned, with special emphasis on field personnel to be utilized; (4) Knowledge in design and construction requirements; (5) Cost control effectiveness; (6) Location of offices supporting project; and (7) Ability to produce all plan documents on a CAD system fully compatible with Autocad. This is an unrestricted procurement. Construction Cost Magnitude is $5million to $10 million. NAICS code of 541310 applies with a size standard of $4.5 million. Submit two (2) copies of SF 330 with appropriate documentation by close of business December 10th, 2009, No bid packages are available. Project is subject to availability of funds.
