Description
This is not a solicitation announcement. This is a sources sought synopsis only. The following information is provided to assist the Department of Veteran Affairs, Northern Arizona VA Health Care System (NAVAHCS), 500 North Hwy 89, Prescott, AZ 86313 in conducting market research of industry to identify potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, or small business) to provide artificial limbs and fabrication. The applicable NACIS code 532291 (size standard of $7.0 Million). The contractor shall provide artificial limbs, fabrication and repair. Contractor currently must have nationally accredited, branch offices or distribution locations, which meets or exceeds the Joint Commission standards, and are physical located within the NAVAHCS catchment area. Each branch office or distribution location, located within the catchment area, must have a minimum of two years experience providing artificial limbs and fabrication. Contractor must have a minimum of 2 year experience of providing artificial limbs and fabrication, be nationally accredited and meet or exceed The Joint Commission Standards. The contractor shall possess all licenses, permits, accreditations, and certificates as required by local, state and federal law at each of the contractors corresponding locations. The four county catchment area includes Yavapai, Coconino, Mohave and La Paz counties. Interested and qualified firms must provide pertinent information and verifiable documentation supporting their assertions of qualification to this sources sought synopsis of having a nationally accredited branch office or distribution location physically located within the catchment area. Response to this notice should also include company name, address, point of contact, size of business pursuant to the following questions: (1) Is your firm a certified Service-Disabled Veteran Owned Small or veteran owned small business or small business? Responses which do not include verifiable documentation will be considered inconclusive. Responses to this notice shall be e-mailed to richard.stockton@va.gov. Telephone responses will not be accepted. Responses must be received via email no later than 3:00 p.m. 11/17/2009. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
