Description
This amendment is to add the following statement: The VA National Acquisition Center will issue a Request for Proposal (RFP) VA-797-09-RP-0044 for the Department of Veterans Affairs for Power Mobility Scooters The contract period will be for one base year plus four one-year option periods. The estimated quantities for the base year and each option year are: Compact 800/year Mid-Size 3-Wheel 3000/year Full-Size 3-Wheel5200/year Full-Size 4-Wheel2500/year If the offeror is not the manufacturer, a signed agreement between the offeror and the manufacturer must be submitted with the offer and remain in effect for the duration of the contract period including the option years. Solicitation No VA-797-09-RP-0044 will be electronically issued in commercial item format in accordance with FAR Part 12 on or about November 30, 2009 with a closing date of 90 days from the solicitation issuance date. The NAICS code for this procurement is 339113. Any future amendments and other miscellaneous documents will be available electronically for download at the following URL: http://www.fedbizopps.gov. No paper copies of the solicitation will be available, and no telephone requests for paper copies of the solicitation will be accepted. Interested offerors are advised to continuously check this URL for any changes to this solicitation. All responsible sources may submit an offer that if timely received, will be considered. SCOOTERS - INTENDED USE BY MODEL "Compact: Controlled outdoor environments (paved surfaces, ADA compliant inclines). Dissassembles for transport in automobile. "Mid-size 3-wheel: Controlled outdoor environments (paved surfaces, mild terrain). Dissassembles. Can be transported by lift on smaller automobile with a Class 2 hitch. "Full-size 3 wheel: Moderate outdoor terrains and/or for a larger, taller adult. Transported by lift on larger automobiles. "Full-size 4-wheel: Moderate outdoor terrains and/or for a larger, taller adult. Transported by lift on larger automobiles. II. STANDARD REQUIREMENTS FOR ALL MODELS A. Approvals & Test Reports "Food & Drug Administration (FDA): All submitted offers must be FDA (510.k) approved with supporting documentation included. Failure to provide evidence of FDA approval will result in the offer being summarily rejected and withdrawn from further consideration. "RESNA Standards: A RESNA test report must be included for each model submitted. Minimum requirements including specific tests required, are listed. Failure to provide a comprehensive RESNA test report or submitting a report which is incomplete or not legible will result in the bid sample being summarily rejected and withdrawn from further consideration. RESNA test reports will be used to confirm compliance with required specifications. Lack of compliance with specifications as listed will result in the bid sample being summarily rejected and withdrawn from further consideration. RESNA test reports will be used to verify and support parameters identified as Evaluation Factors (performance, ease of use, quality). The RESNA test report will also serve as a heavily weighted Evaluation Factor with increased credit assigned to reports that are consistent with current RESNA test methods and reflect testing performed by independent testing facilities. "The RESNA test report will also be used to determine compliance with SPECIFIC REQUIRMENTS FOR INDIVIDUAL MODELS (table below). Failure to provide a RESNA test report will terminate further consideration of the offer. Contact the Contracting Officer, Freddie Beaulieux (freddie.beaulieux@va.gov) for RESNA information. III. STANDARD FUNCTIONS AND FEATURES FOR ALL MODELS The following minimum specifications apply to all models: "Seat and back support: padded & contoured with fold down back support. Dimensions for seat width, seat depth and back height not specified; must be compatible with the device design and appropriate for the identified weight capacity * "Seat Rotation: 90 degrees to both left and right from forward facing drive position "Arm Supports: Flip back, height adjustable, width adjustable "Seat height adjustable across 2 inch span in one inch increments (3 possible settings). "Color choices: 2 choices required - sample submitted in single color; at least one other color sample must be available for review "Batteries: compatible with air travel "Battery charger "Free-wheel mode "Auto-turn off function "User manual "Flat Free Tires "One year warranty including parts, labor and all associated costs. * The full size three wheel and full size four wheel models must include (2) seat sizes that vary in at least one dimension (width and/or depth and/or back height). Both seats must be compatible with device design and appropriate for the identified weight capacity. The sample can be submitted with one seat; dimensions for the second available seat must be documented. IV.SPECIFIC REQUIREMENTS FOR INDIVIDUAL MODELS Contact the Contracting Officer, Freddie Beaulieux (freddie.beaulieux@va.gov) for Specification Requirements. V.REQUIRED OPTIONS "Basket, Cane Holder, Crutch Holder, Oxygen cylinder mount to accommodate E-Cylinder "Power seat elevation is a required option for the Full-Size 3 Wheel and Full-Size 4 Wheel. Minimum vertical excursion is 5" above lowest seat position. Must include lock-out electronics to prevent driving while seat is elevated or RESNA test report must include results with seat in elevated position. "The power control must be mountable on either side of the center column with common hand tools. For either left or right hand mounting, pushing the control handle forward must drive the scooter forward and pulling the control handle back must drive the scooter in reverse.
