Description
ADDENDUM TO STANDARD FORM 1449 STANDARD FORM 1449 The Social Security Administration (SSA), Region 5, has a requirement for medical consultant services in a variety of specialties. These consultant services will include providing oral and/or written consultations resulting from reviews of SSA disability case files, consulting with SSA medical policy staff and/or training other medical consultant contractors. The services involve the review of medical evidence in disability claims filed under Titles II and XVI of the Social Security Act. No patient contact will be involved. The contractor must have the necessary computer skills to read electronic case files, complete electronic forms, use electronic medical and SSA policy reference material, read and answer email messages, type narrative consultations and input electronic case information. The services will be performed at SSA's Chicago Regional Office at 600 West Madison Street, Chicago, IL. Offerors must have a valid, unrestricted license to practice medicine in any one of the 50 United States, the District of Columbia, or Puerto Rico. Offerors must be able to provide at least 50 hours per month between the hours of 6:00 a.m. and 6:00 p.m CST., Monday through Friday. Some travel may be involved. This acquisition is for commercial services prepared in accordance with Federal Acquisition Regulation (FAR) Part 12 and is conducted using simplified acquisition procedures in accordance with FAR Part 13.5. All contract awardees will be required to undergo a security background screening which will include completion of fingerprint cards before beginning performance under the contract. To be eligible for award, offeror must be registered in the Central Contractor Registration database prior to award. For registration information, go to the Central Contractor Registration website at http://www.ccr.gov. Offerors must have a Dun and Bradstreet (DUNS) number to begin registration. The period of performance will be from January 1, 2010 (or date of award through December 31, 20010 with three 1-year renewable option periods. No telephone requests for the RFQ will be honored. This requirement is a 100% small business set aside. Proposals received from other than small business concerns will be deemed unacceptable and will not be considered. The North American Industry Classification System Code (NAICS) is 621112 and the size standard is $10.0 million.
