- This is a combined synopsis and solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 and 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This procurement is being issued as full and open competition. This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-37 and DFARS Change Notice 20091019. The North American Industry Classification System (NAICS) code is 339114 and the size standard is 500 employees. Solicitation number FA3047-09-TC0417 is issued as a request for quotation (RFQ). FAR Clause 52.214-21, "Descriptive Literature" and FAR Clause 52.211-9, "Desired and Required Delivery Time" are applicable to this solicitation. Offerors quoting an or equal must provide illustrations, drawings, or brochures that show the dental units characteristics. Contract Line Item Numbers (CLINs) for this solicitation are as follows: This RFQ is for Brand Name for Standardization Considerations or Equal Item Description Quantity CLIN 0001 - A-dec 511 Dental Chairs 82 ea CLIN 0002 - A-dec 532 Traditional Delivery System with deluxe touchpad 62 ea CLIN 0003 - A-dec 532 Traditional Delivery System with deluxe touchpad (Hygiene) 10 ea CLIN 0004 - A-dec 532 Traditional Delivery System with deluxe touchpad 10 ea CLIN 0005 - Stainless steel trays 82 ea CLIN 0006 - 551 Assistants Instrumentation 82 ea CLIN 0007 - 561 Monitor Mounts 82 ea CLIN 0008 - 1601 Doctor's stool with contoured seat 82 ea CLIN 0009 - 1622 Assistant's stool with contoured seat 82 ea CLIN 0010 - NEOVO 15" LCD Monitor 82 ea NOTE 1: Offerors must provide a trade-in allowance for 82 existing ADEC 1040 dental units currently in use. NOTE 2: The successful offeror shall provide containers for disposal of 82 existing ADEC 1040 dental units. The government will remove the units from the clinics, and place the old units in the containers provided for disposal. Disposal cost must be inclusive of prices quoted. Coordination with the Clinical Engineering unit will be necessary for delivery of the containers, and removal of the ADEC 1040 units from Lackland AFB for disposal. Contact information will be provided to the awardee after award. NOTE 3: Dental treatment units must come preassembled from the factory (excludes LCD monitors and mounting hardware). On-site installation should only require the connection of utilities (electric and air) to make the units functional. NOTE 4: LCD flat panel monitors must be wall mountable. Failure to comply with any of the notes above shall be cause for rejection of your offer. Dental Unit Systems must meet the requirements of the USAF Dental Evaluation and Consultation Services (DECS). Offerors submitted product must have been previously rated by DECS. The following rating descriptions are used by DECS, and will be assigned to offers received as there technical rating: Outstanding - A product that is reliable, user friendly, cost effective, and has superior design and workmanship. Exceeds most applicable standards and requirements. Has clearly identifiable advantages over similar products. Excellent - A product that is reliable, user friendly, cost effective, and free of defects in design and workmanship. Meets applicable standards and requirements. Has identifiable advantages over similar product. Acceptable - A product that is reliable and free of defects in design and workmanship. Meets critical elements of applicable standards and requirements. Performs all essential functions for which it was design. Marginal - A product that performs essential functions but has poor operating features or handling characteristics that renders it difficult to use. May not meet manufacturer's claims of performance. More user-friendly or cost-effective alternatives are available. Unacceptable - A product that fails to perform essential functions for which it was designed. May not meet applicable standards or requirements. May be unsafe to use or is of poor quality rendering it unsuitable for use in Air Force dental facilities NOTE: Only products rated Acceptable or above by the DECS, and that are proven to be an or equal will be considered for award. The DECS rating is significantly more important than price on this acquisition. "Descriptive Literature" FAR 52.214-21, applies to this RFQ. Descriptive literature is required with any submission to ascertain if a product other than Adec is an "or equal." Failure to provide descriptive literature will be cause for rejection of your quote. Additionally, if the descriptive literature fails to show that the product offered conforms to the requirements of the solicitation, the Government will reject the quote. "Desired and Required Delivery Time" FAR 52.211-9, applies to this RFQ. Failure to indicate proposed delivery times may be cause for rejection of your quote. The clause is set forth in full text below for your convenience and completion. Please return a copy of the completed clause with your quote.   Desired and Required Time of Delivery (Jun 1997) (a) The Government desires delivery to be made according to the following schedule: DESIRED DELIVERY SCHEDULE [Contracting Officer insert specific details] ITEM NO. QUANTITY WITHIN DAYS AFTER DATE OF CONTRACT All All 30 Calendar Days If the offeror is unable to meet the desired delivery schedule, it may, without prejudicing evaluation of its offer, propose a delivery schedule below. However, the offeror's proposed delivery schedule must not extend the delivery period beyond the time for delivery in the Government's required delivery schedule as follows: REQUIRED DELIVERY SCHEDULE [Contracting Officer insert specific details] ITEM NO. QUANTITY WITHIN DAYS AFTER DATE OF CONTRACT All All 60 Calendar Days Offers that propose delivery of a quantity under such terms or conditions that delivery will not clearly fall within the applicable required delivery period specified above, will be considered unacceptable and rejected. If the offeror proposes no other delivery schedule, the desired delivery schedule above will apply. OFFEROR'S PROPOSED DELIVERY SCHEDULE ITEM NO. QUANTITY WITHIN DAYS AFTER DATE OF CONTRACT (b) Attention is directed to the Contract Award provision of the solicitation that provides that a written award or acceptance of offer mailed or otherwise furnished to the successful offeror results in a binding contract. The Government will mail or otherwise furnish to the offeror an award or notice of award not later than the day the award is dated. Therefore, the offeror shall compute the time available for performance beginning with the actual date of award, rather than the date the written notice of award is received from the Contracting Officer through the ordinary mails. However, the Government will evaluate an offer that proposes delivery based on the Contractor's date of receipt of the contract or notice of award by adding (1) five calendar days for delivery of the award through the ordinary mails, or (2) one working day if the solicitation states that the contract or notice of award will be transmitted electronically. (The term "working day" excludes weekends and U.S. Federal holidays.) If, as so computed, the offered delivery date is later than the required delivery date, the offer will be considered unacceptable and rejected. (End of Clause) The following clauses and provisions apply to this solicitation: FAR 52.212-1 - Instructions to Offerors - Commercial Items. Addendum: The following paragraph is tailored as follows: Para (c), Period for Acceptance of Quotes. The offeror agrees to hold the prices in its offer firm for 60 calendar days for the date specified for receipt of offers. Interested parties may submit a written offer in accordance with FAR 52.212-1. Offers submitted orally will not be accepted. Your offer must include discount terms, delivery date, tax identification number, cage code, and DUNS number. To be eligible to receive an award resulting from this RFQ, offerors must be registered in the DoD Central Contractor Registration (CCR) database, no exceptions. To register, please apply via the Internet at http://www.ccr.gov or by calling 1-888-227-2423. Offerors must complete the Representations and Certifications in the ORCA database at http://www.bpn.gov/. FAR 52.212-2 - Evaluation - Commercial Items. (Jan 1999) The following factors shall be used to evaluate offers: (1) DESC rating; (2) Conformance with delivery requirements; and (3) Price. The evaluation factors are listed in descending order of importance with the DESC rating carrying significantly more weight than items 2 or 3. Only firm-fixed price offers will be evaluated. The Government will first evaluate the technical acceptability of all offers. To be acceptable and eligible for award an offer must have a DESC rating of acceptable or above. If the technically acceptable offeror with the lowest evaluated price is judged to have an "Outstanding" DESC rating that offer represents the best value for the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. Upon completion of the evaluations, the government will determine which proposal offers the best value as follows: (1) If the technically acceptable offeror with the lowest evaluated price has an overall "Outstanding" DESC rating, that offer represents the best value for the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. (2) If the technically acceptable offeror with the lowest evaluated price does not have an overall "Outstanding" DESC rating, the process will continue (in order by price) until a technically acceptable offeror is found with an overall "Outstanding" DESC rating at which point the government will conduct an integrated best value assessment of that offeror and those lower priced offerors. If no technically acceptable offeror in the competitive range has received an overall "Outstanding" DESC rating, the government will make an integrated assessment best value award decision involving all technically acceptable offerors in the competitive range. In either event, the government will award to the offeror whose proposal offers the best value to the government." The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. All responses that meet the criteria contained within will be considered. FAR 52.212-3 - Offeror Representations and Certifications--Commercial Items FAR 52.212-4 - Contract Terms and Conditions--Commercial Items; Addendum: paragraph (c) Changes is tailored as follows: Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.
