Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NIHDK-00-000 and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-37. The North American Industry Classification (NAICS) Code is 561990 and the business size standard is $7.0. This solicitation is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. The National Institutes of Health (NIH) National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), in providing the services of independent program evaluator on numerous federal, state and local projects. This work has involved evaluation and survey design, development, implementation, analysis and reporting. BACKGROUND: Diabetes was rare among American Indian and Alaska Native peoples until about 50 years ago. Since then, diabetes has become one of the most common and serious illness in the Tribal Nations of North America (Narayan, 1997). In 2002, more than 100,000 American Indian and Alaska Native (AI/AN) adults, or nearly 15 percent of those receiving care from the Indian Health Service (IHS), were estimated to have diabetes. The total prevalence of diabetes is at least three times that for non-Hispanic whites of similar age. Prevalence rates vary by Tribal Nations, rising to 15.9 percent among the Plains Tribes (CDC, 1998). In a new and alarming turn of events, type 2 diabetes, typically considered an adult disorder, is now emerging in all populations of youth in the United States, including AI/AN populations (Narayan et al., 2000). In 2001, the congressionally authorized Diabetes Mellitus Coordinating Committee, chaired by the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), part of the National Institutes of Health (NIH), hosted a meeting with the Indian Health Service Division of Diabetes Treatment and Prevention (IHS/DDTP) and the Tribal Leaders Diabetes Committee (TLDC) to address the seriousness of type 2 diabetes in AI/AN communities. At the meeting, NIDDK accepted the challenge from the TLDC to lead the effort to prevent type 2 diabetes in AI/AN youth by developing a curriculum that would teach the science of diabetes in tribal schools. What followed was a unique, collaborative effort that included NIDDK, IHS/DDTP, the Centers for Disease Control and Prevention Native Diabetes Wellness Program (CDC/NDWP), eight Tribal Colleges and Universities (TCUs), and the NIH Office of Science Education (NIH/OSE). The result of this collaboration is the DETS Health Is Life in Balance K12 curriculum. The DETS Project is part of a national effort to decrease the incidence and improve the care of type 2 diabetes among American Indian and Alaska Natives (AI/AN). The DETS Project is a K - 12 Curriculum that was developed using a multidisciplinary approach. The DETS Curriculum consists of units that incorporate National Science Education Standards, Inquiry-Learning (5E model), and AI/AN cultural and community knowledge. It is the intent of NIDDK to procure the services of Dr. Doug Coulson, as an independent program evaluator. The following tasks are identified: Task Order One: Tribal Colleges and Universities (TCU) recruitment activity and distribution of DETS curriculum materials: Receive TCU 2009 data; Analyze TCU generated self-reports on DETS recruitment activity; Receive Indian Health Service (IHS) data; Analyze geographic distribution of IHS order database activity; Analyze trend information from HIS order database; Produce interim report #1. Task Order Two: Present findings at Steering Committee meeting: Compare TCU recruitment reports with HIS curriculum order database activity; Travel to DETS meeting to present findings. Task Order Three: TCU recruitment activity and distribution of DETS curriculum materials: Receive TCU 2010 data; Analyze TCU generated self-reports on DETS recruitment activity; Receive HIS data; Analyze geographic distribution of HIS order database activity; Analyze trend information from HIS order database; Produce interim report #2. Task Order Four: Curriculum use in schools: Receive DETS teacher web survey data; Analyze web data for extent of curriculum use; Produce interim report #3. Present finding at Steering Committee meeting: Compare curriculum use to recruitment effort; Compare curriculum use to HIS orders; Travel to DETS meeting to present findings. Final integrated report on TCU recruitment activity, distribution of DETS curriculum materials and use of curriculum in schools: Analyze DETS recruitment efforts, HIS orders and curriculum use statistics; Produce final integrated report on findings. Since 1985 Dr. Doug Coulson has worked as an independent program evaluator on numerous federal, state and local projects. This work has involved evaluation and survey design, development, implementation, analysis and reporting. In September of 2003 Dr. Coulson joined the NIDDK Diabetes Education in Tribal Schools (DETS) Project as a evaluator. Since joining the project Dr. Coulson has designed and implemented practical national evaluation efforts for beta testing and field testing the DETS curriculum. In the course of this work he has visited many tribal schools across the United States observing DETS lessons, and interviewing DETS teachers and students. In 2007 Dr. Coulson successfully managed a national field test evaluation involving 60 schools, 100 teachers and 1500 students. In 2006 Dr. Coulson assumed the Chair of the DETS Evaluation Subcommittee. Dr. Coulson is familiar with the Tribal College and University structure, works well with his Native American colleagues and has carried out his DETS evaluation responsibilities in an independent and professional manner. Doug Coulsons role as evaluation expert within the DETS study as well as contractor specifically completing Task Orders for which he billed and was paid, makes him unique to the continuation of the last but ongoing stage of the DETS program which will be to evaluate the effectiveness of the DETS curriculum in the long term and complete process evaluation in the short term. Basis for Award: Offerors must provide descriptive literature or other material that demonstrates their offer meets the foregoing requirements. The contract will be awarded to the responsible offeror whose offer fully meets the requirements of the solicitation, provides the best value, with price and technical factors equally important in award evaluation. The offeror must include a completed copy of the following provisions: FAR Clause 52.212-1 Instructions to Offerors Commercial; As stated in FAR Clause 52.212-2 (a), The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Evaluation, Price, and Past Performance. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions Required To Implement Statues or Executive Orders Commercial Items, Contract Terms and Conditions Commercial Items; and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items Deviation for Simplified Acquisitions. This notice of intent is not a request for competitive quotations however; all responses received within 15 days from the date of publication of this synopsis will be considered by the Government. A determination by the Government not to compete this proposed acquisition is based upon responses to this notice and is solely for the purpose of determining whether to conduct a competitive acquisition. Vendors must submit a listing of the most recent contracts/awards (minimum of 3) in which similar apparatus or equipment was purchased. Contracts/awards may include those entered within the Federal Government, state and local governments and commercial concerns. Include the following information for each contract or subcontract: 1.Name of Contracting Organization,2.Contract Number (for subcontracts provide the prime contract number and the subcontract number,3.Contract Type,4.Total Contract Value, 5.Description of Requirement to include Statement of Work,6.Contracting Officers Name and Telephone Number, and7.Program Managers Name and Telephone Number. * If past performance questionnaire is available, please submit. This notice of intent is not a request for competitive quotations however; all responses received within 15 days from the date of publication of this synopsis will be considered by the Government. A determination by the Government not to compete this proposed acquisition is based upon responses to this notice and is solely for the purpose of determining whether to conduct a competitive acquisition. The offeror must include in their quotation, the unit price, the list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Interested vendors capable of furnishing the government with the item specified in this synopsis should submit their quotation to the below address.
