Description
This is a Request For Quote for Adult Day Center, for the provision of Adult Day Care (ADC) and/or Adult Day Health Care (ADHC) services in support of the VA Medical Center, Spokane, Washington. NAICS:642120. This notice of intent is a request for competitive quotes any firm that believes it can meet the following requirements may give written notification to the Contracting Officer, VAMC Spokane, WA. 4815 N Assembly St, Spokane, WA., within seven (10) days from the date of publication of this synopsis: The adult day health care center must provide the following core services: 1. Skilled nursing services other than routine health monitoring with nurse consultation. 2. At least one of the following skilled therapy services: physical therapy, occupational therapy, or speech-language pathology or audiology. 3. Psychological or counseling services, including assessing for psycho-social therapy need, dementia, abuse or neglect, and alcohol or drug abuse; making appropriate referrals; and providing brief, intermittent supportive counseling. 4. Ambulation. 5. Body Care. 6. Eating. 7. Positioning. 8. Self-medication. 9. Transfers. 10. Toileting. 11. Social Services on a consultation basis. 12. Personal hygiene and bathing at a level that ensures client safety and comfort while in attendance at the program. 13. Routine health monitoring with consultation from a registered nurse that a consulting nurse acting within the scope of practice can provide with or without a physician's order. Examples include: obtaining baseline and routine monitoring information on a client's health status, such as vital signs, weight, and dietary needs; general health education; communicating changes in the client's health status to the client's caregiver; and annual and as needed updating of the client's medical record. 14. A nutritional meal and snacks provided each four-hour period at regular times comparable to normal meal times, including modified diet if needed. 15. Supervision and or protection for clients who require supervision or protection for their safety. 16. Assistance with arranging transportation to and from the program. 17. First aid and provisions for obtaining or providing care in an emergency. 18. General therapeutic activities that an unlicensed person can provide or that a licensed person can provide with or without a physician's order. These services are planned and provided as an integral part of the client's plan of care and are based on the client's abilities, interests and goals. Examples include: recreational activities, diversionary activities, relaxation therapy, cognitive stimulation, and group range of motion or conditioning exercises. A contractor shall have an established facility located within the City Limits of Spokane, Washington.
