Contractor will provide corrugated box (4.0 cubic feet), red bag liner, complete documentation, transportation and disposal. Services shall be provided every 4 weeks at a cost of $55.00 for the first box and $40.00 for each additional box. Cost for the first box and for additional boxes will be increased 6% for each Option Year that is exercised. b.Contractor's contact at VA's CBOC will be Lauren Fair (203) 325-0764. The hours of the CBOC is from 8:00am to 4:30pm, Mondays through Fridays, excluding federal holidays. c.Contractor shall send bills for services provided to the Stamford CBOC to: VA Connecticut Healthcare System Program Assistant (11ACSL) 950 Campbell Avenue West Haven, CT 06516 Please note that the invoice address(es) for services provided to the VA in West Haven and Newington remain unchanged. This address is only for services provide to VA's CBOC in Stamford, CT.
