Description
This is a new solicitation for the purchase of professional services (Physical Therapy Services), in accordance with the Federal Acquisition Regulations (FAR) Part 12.6 as supplemental with additional information included in this announcement for the Department of the Interior (DOI), Bureau of Indian Affairs (BIA), Bureau of Indian Education (BIE), Western Regional Office (WRO). The DOI/BIA-BIE/WRO has a requirement for the purchase of professional services for the Bureau of Indian Education Papago Schools. The services are to be delivered to four schools in Sells, Arizona 85634. This notice is hereby issued as RFQ No. RMH00090047. All responsible sources are encouraged to submit a quote. Please respond to this RFQ, which is effective Friday, September 11, 2009, and will close on December 18, 2009. No other RFQ or Solicitations will be issued for this requirement. Quotes shall be submitted to Geraldine Williams, Purchasing Agent, 2600 North Central Avenue, 4th Floor, Phoenix, Arizona 85004 or emailed to geraldine.williams@bia.gov by the close of business, 4:30pm Local Time, on Friday, December 18, 2009. This is a 100% set-aside for Total Small Business Concern. The NAICS for this acquisition is 621340. SCOPE OF WORK Physical Therapy service for exceptional students at four (4) Bureau of Indian Education schools on the Tohono O'odham Reservation, southern Arizona, beginning January 4, 2010 thru June 30, 2010. The students needing services are schooled at the San Simon Elementary School and Santa Rosa Ranch School. Professional services provided will include direct therapy to students, consultations with instructional staff to include evaluations, participation in IEP meetings as necessary. The contractor will provide services based on the needs described in each student's individualized education program (IEP). The contractor will provide a direct services log for each school and each site visit; and will be asked to meet with the Special Education Coordinator and Business Office at the beginning of the contract. The meetings with the students shall be conducted during regular schools hours (8:00am - 3:00pm) and the contractor shall report to the Principal's Office at every visit. Extended school year services may be provided for specific students during the month of June and/or July, if needed. The contractor will be reimbursed for mileage for trips made to the following schools. SCHOOLS Santa Rosa Ranch School, 48 Miles west of Tucson, Hwy 86 Milepost 131, Sells, Arizona San Simon Elementary School, Hwy 86 Milepost 75 (East of Ajo, AZ) QUOTATION/SUBMITTALS The contractor to provide a "per day" quotation for costs for providing Physical Therapy Services. The per day costs should include evaluations as necessary and travel costs. At this time, there are four (4) students needing services at Santa Rosa Ranch School, and six (6) students at San Simon Elementary School. Your quotations should be based on the ten (10) students. Estimated Not to Exceed 12 Visits Per Student: _____ Days @ $__________ = $_____________ Mileage: ______ Miles Roundtrip. Federal Travel Regulation will apply. Mileage will be reimbursed at the rate of $.55 per mile. Prior to awarding the contract, the BIA-BIE will request copies of current Physical Therapist Licenses for the persons who will be assigned to provide services to the schools. If the Therapists have already received a security clearance, please provide documentation of same. DELIVERY This contract to begin on January 4, 2010 and end June 30, 2010. CONTRACT CLAUSES: This notice hereby issued as RFQ No. RMH00100005. this RFQ will be conducted as a commercial item acquisition in accordance with FAR Part 12. The following provisions and clauses are incorporated and in effect through Federal Acquisition Circular (FAC) 2005-26. The FAR clauses and provisions are icorporated into this acquisition shall be: 52.212-1 Instructions to offerors - Commercial Items (June 2008); 52.212-3; Offeror Representations and Certifications - Commercial Items (June 2008); 52.212-4 Contract Terms and Conditions - Commercial Items (Feb 2007); 52.212-5 Contract Terms and Conditions Required toImplement Statutes or Executive Orders - Commercial Items (June 2008).
