Price List Statement of Work Combined Synopsis/solicitation: PURCHASE MEDICAL WASTE DISPOSAL SERVICE. Request for quote due 21 DECEMBER 2009, 12:00 Noon Eastern Standard Time (EST). Point of Contact: A1C PANUWAT SRIAUTHARAWONG, 301-981-6088, Contract Specialist. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are requested and a written solicitation will not be issued. This combined solicitation/synopsis reference number is F1A4A09245A001-MEDICAL_WASTE_DISPOSAL and is being issued as a combined synopsis/solicitation. This combined solicitation/synopsis document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-37. This acquisition is 100% set-aside for small businesses. The associated NAICS code is 562211 with a $10,500,000 size standard. The contractor shall submit a quote for following items (Medical Waste Disposal Service): Line Item: 0001, Medical Waste Disposal Service See Attached Performance Work Statement for Additional Information. Period of Performance: Base Year: 1 January 2010 - 30 December 2010 Option Period One (1): 1 January 2011 - 30 December 2011 Option Period Two (2): 1 January 2012 - 30 December 2012 Option Period Three (3): 1 January 2013 - 30 December 2013 Option Period Four (4): 1 January 2014 - 30 December 2014 NOTE: Proposals should include all items listed in the solicitation. Award will be made on an all or none basis.
