Description
NRC HEALTH CENTER LAB AND OTHER TESTS 2008 HEALTH CENTER STATISTICS NRC SECURITY FORM Performance Requirements Summary Government Furnished Equipment solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement along with the attachments constitutes the only solicitation. The solicitation number is RS-HR-09-727, and it is being issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-34, effective 01 Jul 09. NAICS 511120 applies. This acquisition is being conducted under full and open competition. The subcontracting plan requirements of P.L. 95-507 are applicable to this contract; thus, all large business offerors must submit a detailed subcontracting plan with their proposal. The U.S. Nuclear Regulatory Commission intends to award a fixed unit price requirements type contract for providing technical assistance to employee health services identified in the attached solicitation. The period of performance shall consist of one base year and four (4) one-year options. The following FAR provisions and clauses apply to this solicitation and are incorporated by reference: 52.212-1 Instructions to Offerors-Commercial Items; NRC ADDITIONAL INSTRUCTIONS TO OFFERORS To respond to this request, your organization will be required to provide an electronic or hard copy proposal, which consists of a written technical proposal and a price/cost proposal. Number of copies: One original and one copy of your technical and price/cost proposal. The offeror is also required to submit a copy of their proposal via email to kala.shankar@nrc.gov. Electronic Format - electronic versions of proposals shall be formatted with the products contained in Microsoft Office Professional 2003. Pricing spreadsheets shall be presented in Microsoft Excel 2003. NRC is not responsible for electronic difficulties and/or administrative problems associated with the transmission of any files to the NRC. PDF format is authorized and the documents may be sent by e-mail to kala.shankar@nrc.gov no later than the time and date established for submission. CLOSING DATE No later than 4:00 pm EST on January 14, 2010. Proposals delivered by hand, including delivery by any express mail services or special delivery services which use a courier or other person to deliver the responses in person to the NRC, should be addressed to U.S. Nuclear Regulatory Commission Division of Contracts 11555 Rockville Pike ATTN: Kala Shankar, Senior Contract Specialist - Mail Stop, TWB-01-B10M RFQ No. RS- HR-09-727 Rockville, MD 20852-2738 SUBMISSION OF QUESTIONS It shall be the obligation of the offeror to exercise due diligence to discover and to bring to the attention of the Contracting Officer (CO) at the earliest possible time, any ambiguities, discrepancies, inconsistencies, or conflicts between the statement of work and other documents attached hereto or incorporated by reference herein. All questions shall be e-mailed to kala.shankar@nrc.gov no later than 5:00 pm EST January 7, 2010. The closing date will not be extended for the purpose of responding to questions received after this date. DISCLAIMER RFQ RS-HR-09-727 will be distributed solely through the FedBizOpps website (http://www.fedbizopps.gov) The FedBizOpps website provides downloading instructions. All future information about this acquisition, including solicitation amendments will also be distributed solely through the FedBizOpps website. Hard copies of the solicitation document and its related documents will not be available. Because this RFP will be posted on this website, interested parties are instructed NOT to call, send letters, e-mails or faxes requesting the RFP. Interested parties are responsible for monitoring the FedBizOpps website to assure that they have the most up-to-date information about this acquisition. The NRC requires the prospective awardee to be registered in the Central Contractor Registration (CCR) database prior to award, during performance, and through final payment of the resulting contract in accordance with Federal Acquisition Regulation, 52.204-7, Central Contractor Registration. Processing time, which normally takes 48 hours, should be taken into consideration when registering. Potential Offerors who are not registered should consider applying for registration immediately if interested in responding to this solicitation. Potential Offerors may obtain information on registration and annual confirmation requirements via the internet at http://www.ccr.gov. The Government will not reimburse interested parties for any costs associated with responding to this business opportunity. Issuance of this combined synopsis/solicitation does not constitute an award commitment on behalf of the NRC. The NRC reserves the right to reject any and all offers received.
