Description
Amendment 002 The Federal Bureau of Prisons, Privatized Corrections Contracting, Office, Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient substance abuse treatment services and mental health treatment services, within a Six-Mile radius from, 300 S. Adam Street, Tallahassee, Florida 32301 and be accessible to local public transportation. Performance periods under this contract will be for a one-year base period estimated to begin on February 1, 2010 with four (4) one-year option periods, with service ending (if all options years are renewed) on January 31, 2015. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. Base Year: Estimated number of inmates 20: Intake Assessments (one per inmate) for 1 inmates = 1 assessments; Individual Counseling Sessions ( 30 minutes = 1 unit), 48 units (per inmate) x 20 inmates = 960 units; Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 20 = 1,440 units; Family Counseling Sessions (30 minutes = 1 unit) 12 units (per inmate) x 6 = 72 units; Psychiatric Evaluation & Report (one per inmate) for 6 inmates = 6 Evaluations; Mental Health Intake assessment and report (one assessment and report per inmate) for 3 inmates = 3 Reports; Mental Health Individual Counseling 48 units per inmate for 9 inmates = 432 units; Medication Monitoring 1 report per inmate for 6 inmates = 6 Monitorings. The services shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work dated March 2009. The request for quotation will be available on or after November 30, 2009 and will be distributed solely through the General Services Administrations Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the quotation will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about December 11, 2009. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible.
