VA may determine that an offer is unacceptable if the monthly or hourly rates (for base year and/or option year) are significantly unbalanced. Statement of work for the COMS Audit for the West Haven VA (3) High Pressure Boilers and Newington (3) High Pressure Boilers a total of (6) The contractor will be responsible for doing a COMS Audit as it pertains to the requirements of the State of Connecticut EPA. The contractor will meet the requirements for the monitoring of the air quality of spent fuel gases to the environment. The contractor will perform preventative maintenance and repair of the recording equipment, sensors and associated equipment used for monitoring the Opacity of the spent fuel gases of the Boilers in the Bldg#16 Boiler Plant. Additionally the contractor will submit reports to the State of Connecticut and VA as required per State of Connecticut DEP and EPA requirements. Copies of records will be kept by the VA and the contractor of the records of the monitoring equipment and devices of the Opacity monitoring system Contractor questions or comments shall be directed to Barry Beauvais at (203) 932-5711 x3216. Service Records and Reports- Vendor shall provide original copies of all service records and reports on a form approved by or provided by VA. Service reports shall include hours and names of employees and shall include all parts and material used. Failure to provide a fully completed report is basis for non-payment of invoice. Service reports shall be reviewed by and signed by Facilities Management Service Representative upon completion of work and prior to the service technicians departure from the station.
