- Attachment 4 - Instructions Attachment I - Price Proposal Attachment 5 - Factors for Award Attachment 2 - Statment of Work Attahcment 3 - Clauses This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisitions Regulation (FAR) Part 12 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation issued as a Request for Proposal (RFP)). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 05-26. The General Services Administration (GSA) has a need for System Diagnostic on their Extron Equipment coordinated through the Office of the Chief Information Officer located at GSA's Headquarters in Washington, DC. All potential offerors must be registered in the Central Contract Registration (CCR) prior to award ( www.ccr.gov ). The associated North American Industrial Classification System (NAICS) code for this procurement is exclusively for Small Businesses under NAICS 423430 size standard in number of employees (100).. Price Proposal: Please refer to Attachment 1, Price Proposals. The Contractor shall complete Attachment 1. All price proposals shall be on a fixed price basis. Statement of Work: Please refer to Attachment 2 Statement of Work. Attached is the Statement of Work (SOW) for the Request for Proposal (RFP) providing guidance and information for this requirement. The period of performance is a base year and two option years. The Government intends to solicit and award only one firm fixed price contract. All potential contractors must have office located within a fifty-mile radius of Washington, DC. Clauses: Please refer to Attachment 3 for Commercial Items and Additional Clauses Submission of Questions, Site Visit and Proposals: Please refer to Attachment 4, Instructions, Conditions, and Notices to Offerors or Respondents The Contractor must submit Questions NO LATER THAN 9 :00PM, DST, Monday, December 21, 2009. The Site Visit shall be held at GSA Headquarters on 9:00AM DST, Tuesday, December 22, 2009. The contractor shall submit their final proposal on or before 7 :00AM, DST, Thursday, December 24, 2009. Proposals must be submitted by email to Ms. Doris Williams at: doris.williams@gsa.gov. Evaluation Criteria : Please refer to Attachment 5, Evaluation Factors for Award. Proposals will be evaluated based on best value to the government. When combined, technical is significantly more important than price. Contractors must submit their final proposals for evaluation.
