Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation prepared in accordance with the format contained in FAR Subpart 12.6. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. A QUOTE IS REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED, SEPARATELY. Quotes shall reference Request for Quotes (RFQ) number W911RZ-10-T-0050. This announcement incorporates provisions and clauses in effect through Federal Acquisition Circular 05-38. It is the Contractor's responsibility to be familiar with all applicable clauses and provisions. Fort Carson intends to award a firm-fixed-price contract for Tactical Fiber Optics Repair Kit and Training, brand name FORCE001. This requirement is being solicited as a 100% Service Disabled Veteran Owned Small Business (SDVOSB) set aside for brand name only. The NAICS code for this procurement is 334417 with a small business size standard 500 employees. Quotes shall include pricing of: CLIN; ITEM DESCRIPTION;QTYU/I 0001; Tactical Fiber Optics Repair Kit; 2ea Brand name only -- FORCE001 0002 ; FORCE001 Training Seats (8 12 Feb 10); 10 ea (see statement of work for training course requirements) STATEMENT OF WORK 1. General. The contractor shall provide two Fiber Optic Repair Kits (Brand Name FORCE001) with the capability to repair military grade tactical fiber optic cable (TFOCA) and 40 hours of fiber optic repair training for 10 soldiers. 2. Training Course: The vendor shall provide a 40 hour course covering the specific use of the fiber optic repair kit, focusing on the repair of TFOCA, TFOCA II, SC, and ST connectors and multi-strand fiber splicing. Successful completion of the five day course and test/exam shall qualify the student to receive a certification from Amphenol Fiber Systems International on the TFOCA-II connector. Primary means of instruction shall be hands-on, with supplies provided as part of the cost. Topics should include: Introduction to Fiber Optics, Fiber Optic Advantages, Fiber Optic Essentials, Light Guiding Principles, Fiber Optic Cables, Fiber Optic Connectors, Fiber Optic Safety, ST Assembly, TFOCA-II Receptacle Assembly, Student ST-ST Cable Assembly, Assembly of TFOCA-II Plug, Troubleshooting and Testing Methodologies of all Assemblies, Test equipment Utilization and Trouble Shooting Techniques, Troubleshooting and Testing Methodologies of all Assemblies and Mechanical and Fusion Splicing and Splice Enclosures. End of SOW. FOB Destination Training Location (Fort Carson, CO 80913) Delivery Date: 7 days ARO with training to be conducted from 8 12 Feburary 2010. Interested parties are encouraged to use alternative dispute resolution procedures to the maximum extent practicable, in accordance with the authority and requirements of the ADR Act. EMERGENCY MEDICAL TREATMENT Emergency medical treatment will be provided by Evans Army Community Hospital in order to prevent undue suffering or loss of life. Emergencies will be treated only during the period of emergency and appropriate action will be taken to discharge or transfer such patients as soon as the emergency period ends. Charges for medical care will be at the prevailing rate and at the Contractor's expense. FIRE PREVENTION The Contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association in the National Fire Code and other recognized fire prevention agencies and post regulations. OPERATION OF PRIVATELY OWNED VEHICLES Contractor employees operating privately owned vehicles on Fort Carson shall be required to comply with all applicable traffic laws, parking restrictions, and restricted area requirements (FC Suppl. No. 1 to AR 190.5) To be eligible for an award, contractor must be registered in the Central Contractor Registration (CCR) database. A contractor can contact the CCR by calling 1-888-227-2423 or e-mail at http://www.ccr.gov. NO EXCEPTIONS. A DUNS (Dun and Bradstreet) number is required in order to register. All invoices shall be submitted through Wide Area Work Flow. Register at https://wawf.ef.mil/index.html.
