- W912PQ-10-T-0013 Mobile Vision & Audiology Services The United States Property and Fiscal Office for New York (USPFO-NY) proposes to procure Optometric and Audiologic Evaluations for Soldiers participating in an Individual Medical Readiness (IMR) assessment event conducted at Camp Smith Training Site, Cortlandt Manor, NY 10567 on 23 January 2010. BACKGROUND: VISION: Deploying Soldiers must be in Vision Readiness Class (VRC) One or Two. Those who are in VRC Three (need corrective lenses to achieve a minimum distant binocular visual acuity of 20/40) are required to be examined optometrically to generate a prescription to correct refractive error and be measured for corrective lens frames and protective mask inserts. Soldiers in VRC Four are in need of annual vision screening. HEARING: Audiologic health is vital for Force health Protection and Soldier Readiness. Failure to achieve proper testing and documentation at specific levels of audiologic function could preclude a soldier from deploying. Audiologic Health, Readiness and Protection is a command responsibility. The Department of Defense (DoD) has established a uniform Hearing Readiness Classification System. Readiness requires that the date of an audiogram performed through the Defense Occupational and Environmental Health Readiness System – Data Repository (DOEHRS-DR) be recorded in the Medical Protection System (MEDPROS). Pre-mobilization preparation requires a DOEHRS-DR audiogram be done within one year of deployment. In accordance with the All Army Action (ALARACT) 163/2006, dated 6 September 2006, policy now requires all Soldiers with an H2 designation on the physical profile (PULHES) have a complete audiologic evaluation and permanent profiling. Those with an H3 require audiologic evaluation, Speech Recognition in Noise Test (SPRINT), permanent profiling, a Military Occupational Specialty (MOS) Medical Review Board (MMRB) for classification, coding and retention. At the present time, members of the Army National Guard do not require annual screening audiograms. The contractor is required to provide all material, labor, supervision and means required to perform the following: 1.Visual screening exams - projected 20-25 each 2.Optometric exams in order to produce lens and frames for Soldiers in VRC 3 - projected 21 each 3.Annotate Data on Department of Defense (DD) Form 771 4.Complete all demographic information requested on DD Form 771 5.Provide timely feedback and work progress/status as to the number of optometric examinations completed 6.Perform approximately 50 screening audiograms 7.Provide audiologist consultation/evaluation of soldiers with H2 designator or worse 8.Provide SPRINT testing on a projected 10 soldiers expected to reveal a PULHES H3 designation See Statement Of Work for performance specifications. It is intended that the contract be awarded to one vendor.
