- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Contractor shall provide mock survey consultants (One Physician-One RN) certified in Veterans Administration (VA) joint commission accreditation standards to perform the following services for John J. Pershing VAMC in Poplar Bluff, Missouri. 1)Provide an on-site comprehensive pre-survey according to Veterans Administration Joint Commission Standards for this rural hospital system with the following: a.Two Consultants for 1 days specializing in Home Care. b.Two Consultants for 1 days specializing in Long Term Care. c.Two Consultants for 2 days specializing in Acute Care. 2)Assess the John J Pershing VAs, and one Community Based Outpatient Clinic (CBOC), current compliance with applicable standards in the Joint Commission Comprehensive Accreditation Manuals which include Acute Care, Long Term Care and Home Care. 3)Assess organization approach in areas such as leadership, patient safety, infection control, medication management, use of data, environment of care, and medical staff through on-site survey. 4)Utilize tracer methodology for the evaluation of critical focus areas and direct care processes, across services and disciplines in key clinical areas and the environment. 5)Provide systems analysis of strategic and significant topics essential to the provision of care and service and related to quality and patient safety. 6)Provide consultative and educational information as identified during the on-site review and as requested by the organization. 7)Provide pre-consultative planning for identification of Priority Focus Processes, Priority Focus Areas and Clinical Service Groups to guide the development of the on-site review. 8)Validation of the organizations implementation of its plan of action and measure of success. 9)Provide a detailed written report within 15 days of the consultative services, to include the findings and recommendations for each area (Home Care, Long Term Care, and Acute Care). This report should differentiate findings and recommendations for each area. 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition.
