- PWS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Air Force 711 Human Performance Systems Wing (711 HPW/PKA), has a requirement for On Call Hyperbaric Chamber Services to be performed on the hyperbaric chambers located at Wilford Hall Medical Center, Lackland AFB, TX and Brooks-City Base TX. The period of performance will be one year and the total number of hours will be 1240 hrs. The contractor shall perform services as follows: Monitors patients inside the chambers during hyperbaric treatments as the chamber operator. Assists in patient treatment process and disposition. Coordinates with medical and maintenance personnel on chamber equipment configuration and modification for hyperbaric treatment and hyperbaric medicine research. Serves as an inside tender in mutiplace chambers as needed. Orchestrates treatment schedules for hyperbaric research patients including subject transportation and assists research coordination including, but not limited to, physician referrals, records management, Institutional Review Board requirements, and data collection. FAR Part 12, Acquisition of Commercial Items, applies to this acquisition. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38 and Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 20091229. FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. There are no addenda to the provision. FAR 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: Price and Delivery. Delivery is of equal importance to price. IAW FAR 13.106-2(b), award will be made to the offeror that represents the best value to the government considering price and delivery. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, along with Alternate I. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. Under para (b), the following provisions and clauses apply: 52.222-19, Child Labor--Cooperation with Authorities and Remedies; 52.222-26, Equal Opportunity; 52.225-13, Restrictions on Certain Foreign Purchases; 52.232-33, Payment by Electronic Funds Transfer--Central Contractor Registration The clause at DFAR 252.212-7001 Contract Terms and Conditions Required to Implement Statues or Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this acquisition.
