Description
The Department of Veterans Affairs Caribbean Health Care is seeking potential sources capable of providing preventive maintenance, repairs, materials and sanitation services to the Water Treatment System located at the Hemodialysis and Laboratory Services, San Juan, Puerto Rico. Following are the Culligan Equipment: Agua Summa II, Model 32; PV 12D; PV 12R; PVE 120 and MP 4500 (Reverse Osmosis). Contractor shall ensure that the equipment functions are in conformance with the latest published edition of NFPA-99 and OSHA and in accordance with the conformance standards. Contractor shall cover parts and labor for Preventive Maintenance Services including all filters, media components, carbon, sand and resin as needed. Services shall include routing PM service, cleaning, inspection, lubrication, adjustment, calibration, replacing parts, operational tests and routine diagnosis to maintain equipment in an operational status. Contractor shall respond to all repair service for routine priority maintenance within four (4) hours after an authorized repair call notification was made. Emergency repair service, Contractor shall respond within two (2) hours after notification. Contractor shall establish and maintain a Quality Control Program (QCP) in accordance with the Federal Acquisition Regulations. Contractor shall comply with Joint Commission Accreditation of health Care Organization (JCAHO) requirements. NO FORMAL SOLICITATION DOCUMENT EXISTS AT THIS TIME. Responses to this SOURCES SOUGHT NOTICE should include name and address of Company; business size; identify whether or not firm falls into any of the following categories: Service Disabled Veterans-Owned Business (SDVOSB); Veteran-Owned; Hubzone, 8(a); Women-Owned, etc.; experience in providing Preventive Maintenance and Repair to the Culligan Equipment which demonstrates ability to meet the above requirements.
