- The U.S. Forest Service, Washington Office, Job Corps Acquisition Support Branch intends to issue a solicitation for the acquisition of Physician Services at the Wolf Creek Civilian Conservation Center located at 2010 Opportunity Lane, Glide, Oregon 97443. Services shall be provided on-site at the Wolf Creek Civilian Conservation Center and include assisting in the development of a self health care program, orientation of new students, entrance physical exams, consultation with Center Director, and coordination with other Center Health Care Specialists. Personnel performing the services must be certified and/or licensed in the State of Oregon and a copy of Medical License and Drug Enforcement Agency (DEA) number must be provided. The solicitation is anticipated to be issued on or about February 13, 2010. Since the aforementioned date is an anticipated date and not an actual date, interested parties shall monitor the website www.fbo.gov frequently to find the latest information. The requirement is 100% set-aside for small business. The NAICS code is 621111 and the small business size standard is $10 million. The requirement will be solicited and awarded in accordance with Federal Acquisition Regulation PART 12—ACQUISITION OF COMMERCIAL ITEMS Contractors must be registered in the Central Contractor Registration Database located at http://www.ccr.gov, and must complete electronic representations and certifications on the ORCA database located at http://orca.bpn.gov to be considered for contract award. All responsible sources may submit a quote for consideration. The base period of performance is July 01, 2010 through June 30, 2011, with four 12-month option periods. The Government intends to award a firm fixed-price contract resulting from the solicitation, with or without discussions, to the responsible quoter whose quote, conforming to the solicitation, is most advantageous to the Government based on the evaluation factors contained in the solicitation. This pre-solicitation notice does not constitute a solicitation. The solicitation will be posted to this website for downloading and will provide all necessary information for proposals to be prepared and submitted for consideration. Potential quoters shall monitor the website www.fbo.gov for the solicitation and amendments, if any. Paper copies of the solicitation will not be issued; telephonic and facsimile requests for the solicitation will not be accepted. Quotes shall be sent to the attention of Donald R. Watson, Contract Specialist, U.S. Forest Service, 740 Simms Street, Golden, CO 80401, Telephone number: 303-275-5158. Electronic quotes are acceptable. Facsimile proposals are not acceptable. The Government reserves the right to cancel the requirement and/or any resultant solicitation. All questions related to the solicitation must be submitted in writing to Donald R.
