- This is a posting for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442792870312. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38, effective 10 Dec 2009, DCN 20100120, and AFAC 2009-1030. This acquisition will be solicited as unrestricted. The North American Industry Classification System (NAICS) code is 334510. The business size standard is 500. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3845. This request for quotation consists of the following: Tattoo Removal Laser 1 each, with the following minimum requirements: Must be able to remove tattoos of all colors. Must be able to be used for removal of various types of hyperpigmentation, including Nevus of Ota/Ito, mincycline hyperpigmentation, benign pigmented lesions, Café au Lait, scars, freckles, sun and age spots and birthmarks. The following wavelengths are required for this laser: Q-Switched 1064, 755 and 523 nanometer. Protective glasses for physicians and patients for the following wavelengths: 1064, 755 and 523 nanometer. Any associated accessories for the laser and wavelengths to include attachments, handles, aiming equipment, cables, and protective gear. All required software, pamphlets, CD's, operator manuals, safety signs and stickers. Physician training is required and the cost should be included in the quote. Five year warranty for all items. Award will be made in aggregate, all or none. Include any shipping costs to Travis AFB, CA 94535.
