STATEMENT OF WORKPROVISION OF LOCAL EXCHANGE CARRIER DEPARTMENT OF VETERANS' AFFAIRS OFFICE OF INFORMATION & TECHNOLOGY FIELD OFFICE ALBANY, NY February 3, 2010 A. GENERAL INFORMATION 1.Background: The Department of Veterans Affairs, Office of Information & Technology (OI&T) Field Office, Albany, NY is one of ten national offices who provide developmental and application support to computer programs run by the Department of Veterans Affairs Health Administration nationwide. 2.Scope of Work 2.1.Contractor shall provide local dial tone service, including trunking services on contractor provided T-1 lines (36 channel digital), and Direct Inward Dial (DID) services, for the Albany Office of Information, 4th Floor, 113 Holland Avenue, Albany, NY 12208. Service shall also include the support of three (3) T-1s of 36 digital channels each with 300 DID #s. Contractor shall use the existing phone numbers. Support service shall be based on technology that has critical component redundancy as well as automatic system re-initialization to the identical level of service prior to need for reload. 2.2.Place of performance Albany Office of Information & Technology 4th Floor 113 Holland Avenue Albany, NY 12208 2.3.Hours of Contractor Operations: 2.3.1.The following are Federal holidays are observed by the VA and affect the contractor's ability to access to the facilities: New Years DayJanuary 1st Martin Luther King's Birthday3rd Monday in January President's Day3rd Monday in February Memorial DayLast Monday in May Independence DayJuly 4th Labor Day1st Monday in September Columbus Day2nd Monday in October Veteran's DayNovember 11th Thanksgiving Day4th Thursday in November Christmas DayDecember 25th 3.Specific Performance Requirements Expected service requirements include the following: 3.1.Provide local dial tone service including trunking services on contractor provided T-1 lines (36 channel digital); 3.2.Direct Inward Dialing for 300 DIDs; 3.3.Provide 24 hour service 7 days a week 3.4.Provide single point of contact for trouble calls 3.5.Reserve block of consecutively-numbered DID or reserve and maintain existing blocks already under reservation with current LEC; 4.Performance Measures: The Government will evaluate contractor performance and deliverables against the critical performance objectives provided in the tables below to determine if the services rendered are acceptable for payment. Payment will be made on a monthly basis as identified in the Contract Line Item Number (CLIN) of the Schedule. 4.1. Services will be accepted on a monthly basis by certification of the monthly invoice. All required deliverables and performance objectives must be fulfilled in order to obtain acceptance. Acceptance will constitute 100% payment. Non-acceptance may result in negotiation of a credit commensurate with the negative impact on the Office Of Information Field Office, Albany NY mission resulting from the reduced level of performance. Said credit will be negotiated at the time of non-acceptance. Failure to agree on a credit amount will be considered a dispute in accordance with the Disputes Clause of the contract action. Performance Objective Performance StandardMonitoring Method Provide local dial tone service Un-interrupted 24 hours a day 7 days a week 100% of the timeCOTR review of complaints and observance of performance. 5. Government Furnished Property and Facilities: The VA will provide access to contractor point of entry, extended demarcation point and termination point at Avaya S8700 PBX as is reasonable to fulfill the requirements of this SOW. 5.3.Non-Disclosure: Contractor personnel will have access to some privileged and confidential materials of the United States Government. These printed and electronic documents are for internal use only, are not to be copied or released without permission, and remain the sole property of the United States Government. Some of these materials are protected by the Privacy Act of 1974 (revised by PL 93-5791) and Title 38. Unauthorized disclosure of Privacy Act or Title 38 covered materials is a criminal offense. 5.4.The Contracting Officer will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this contract. The Contractor shall release no information. Any request for information relating to this task order presented to the Contractor shall be submitted to the Contracting Officer for response. 5.5.The Contractor recognizes that in the performance of this contract the Contractor may receive or have access to sensitive information, including information provided on a proprietary basis by carriers, equipment manufacturers and other private or public entities. The Contractor agrees to safeguard these privileges and use the information exclusively in the performance of this contract. The Contractor shall follow all Government rules and regulations regarding privacy and information security to prevent disclosure of sensitive information to unauthorized individuals or organizations. 5.6.Indemnification. The Contractor shall save and hold harmless and indemnify the Government against any and all liability claims, and cost of whatsoever kind and nature for injury to or death of any person or persons and for loss or damage to any Contractor property or property owned by a third party occurring in connection with or in any way incident to or arising out of the occupancy, use service, operation, or performance of work under the terms of this contract action, resulting in whole or in part from the acts or omissions of the Contractor, any subcontractor, or any employee, agent, or representative of the Contractor or subcontractor. 5.7.Government's Liability. The Government shall not be liable for any injury to the Contractor's personnel or damage to the Contractor's property unless such injury or damage is due to negligence on the part of the Government and is recoverable under the Federal Torts Claims Act, or pursuant to other Federal statutory authority.
