- Request for Information (RFI)/ Sources Sought inquiry into your organizations interest in providing unattended Home Sleep Study Services with twenty four (24) hour a day, seven (7) days per week telephone support services for VA patients referred by the Sheridan VA Medical Center located in Sheridan WY. The VA contemplates a single award, Firm Fixed Price contract to provide these services. The services shall be provided at the patient's residence therefore the contractor will be required to cover a large geographic area that can include Wyoming, Montana, South Dakota, Idaho, Utah, Nebraska, and Colorado. All offerors must currently be in the business of providing sleep study services and have employees on staff that are Board Certified Sleep Specialist or individuals who fulfill the eligibility criteria for the ABMS sleep medicine certification examination practitioner(s). Dependent upon responses, the proposed contract award priority shall be to Service Disabled Veterans Owned Small Businesses, Veteran Owned Small Businesses then Small Businesses. The period of performance for this contract will be one year, with additional four-one year option periods. The contract base year is estimated to begin in May 2010. Solicitation documents will be available electronically on or about March 12, 2010 at the FedBizOpps website at http://www.fedbizopps.gov. Interested firms are reminded that in accordance with FAR Part 4.11, prospective contractors are required to have an active registration in the Central Contractor Registration (CCR) and FAR Part 4.12, prospective contractors must complete electronic on line annual representations and certifications (ORCA) databases prior to award of a contract. Both can be accessed at the Business Partner Network (BPN) website at http://www.bpn.gov/. Again, this is only an inquiry, no quote or proposal is requested at this time. As such, if your organization has the capability to meet these requirements and is interested, please reply back to me with a statement describing your capabilities. The statement should include verification that your organization is currently in the business of providing sleep study services. This information shall be provided no later than 5:00 PM Mountain Time February 22, 2010.
