- Pre-solicitation Notice:Intent to Issue Basic Ordering Agreements with Adult Day Health Care Providers. The Department of Veterans Affairs, Portland VA Medical Center (PVAMC) (IAW authority under 38 USC 1720 - governing the transfer of Veterans from a VA Facility to a Nursing Home Care; Adult Day Health Care Provider) intends to issue individual Basic Ordering Agreements (BOAs) to the following care providers: OREGON Beaverton Lifeworks NW** 14255 SW Brigadoon Ct Beaverton, OR 97005 Silver Years 9958 SW Artic Dr Beaverton, OR 97005 Sunshine Adult Day Services LLC 2270 SW 198th Ave Beaverton, OR 97005 Canby Thelma's Place 352 NW 2nd Ave Canby, OR 97013 Corvallis Grace Center for Adult Day Services 980 NW Spruce Ave Corvallis, OR 97330 Hillsboro Homeward Bound P.O. Box 1113 Hillsboro, OR 97113 Portland Cognitive Enhancement Center, Inc 15705 SE Powell Blvd Portland, OR 97236 Emerson House Adult Day Program 3577 SE Division St Portland, OR 97202 Salem House on Haven Hill 555 Misty Hill Lane SE Salem, OR 97306 Portland VOA-Lambert House** 2600 SE 170th Portland, OR 97239 VOA-Marie Smith Center** 4616 N Albina St Portland, OR 97217 Tigard Gentog Adult Day Center LLC 11535 SW Durham Rd Tigard, OR 97224 WASHINGTON Vancouver CDM Adult Day Services** 2409 Broadway Vancouver, WA 98663 Innovative Services NW** 9414 NE 4th Plain Rd Vancouver, WA 98662 These BOAs will be for Adult Day Care (ADC) SERVICES which entail, among other things, the following services: The adult day care center must provide the following core services: Ambulation; Body Care; Eating; Positioning; Self-medication; Transfers; Toileting; Social Services on a consultation basis; Personal hygiene and bathing at a level that ensures client safety and comfort while in attendance at the program; Routine health monitoring with consultation from a registered nurse that a consulting nurse acting within the scope of practice can provide with or without a physician's order. Examples include: obtaining baseline and routine monitoring information on a client's health status, such as vital signs, weight, and dietary needs; general health education; communicating changes in the client's health status to the client's caregiver; and annual and as needed updating of the client's medical record; A nutritional meal and snacks provided each four-hour period at regular times comparable to normal meal times, including modified diet if needed; Supervision and or protection for clients who require supervision or protection for their safety Assistance with arranging transportation to and from the program; First aid and provisions for obtaining or providing care in an emergency; and General therapeutic activities: a physician's order is required for licensed persons; unlicensed person can provide therapeutic activities without a physician's order. These services are planned and provided as an integral part of the client's plan of care and are based on the client's abilities, interests and goals. Examples include: recreational activities, diversionary activities, relaxation therapy, cognitive stimulation, and group range of motion or conditioning exercises. and - Adult Day Health Care (ADHC) SERVICES which entail among other things, the following services: The adult day health care center must provide the following core services: Skilled nursing services other than routine health monitoring with nurse consultation; At least one of the following skilled therapy services: physical therapy, occupational therapy, or speech-language pathology or audiology; Psychological or counseling services, including assessing for psycho-social therapy need, dementia, abuse or neglect, and alcohol or drug abuse; making appropriate referrals; and providing brief, intermittent supportive counseling; Ambulation; Body Care; Eating; Positioning; Self-medication; Transfers; Toileting; Social Services on a consultation basis; Personal hygiene and bathing at a level that ensures client safety and comfort while in attendance at the program; Routine health monitoring with consultation from a registered nurse that a consulting nurse acting within the scope of practice can provide with or without a physician's order. PVAMC intends to award these agreements with a Base Year and 4 Option Years with firm-fixed-prices for hourly per diem rates. NAICS code 624120, Size Standard is $7M. This purchase will be under the Simplified Acquisition Procedures as authorized under FAR 13.500. A Request for Quotations (RFQ) will be issued on or around Feb 18, 2010. Interested organizations may submit quotes. Quotes must contain sufficient literature to the extent necessary to allow the government to determine they possess the facility and personnel requirements to perform the services. Quotes must also include the requested pricing, documents, and details as identified and/or requested in RFQ Statement of Work (SOW) and Clauses 52.212-1 "Instructions to Offerors" and 52.212-2 Evaluation Criteria". The anticipated award date is March 31, 2010. Quotations should be submitted in writing to the Contract Specialist (email: dennis.riches@va.gov). Quotations must include a signed/dated SF1449 and signed SF30 Amendments if any are issued. Quotations must be received by the closing date and time identified on the RFQ. Oral communications are not acceptable in response to this notice.
