- Service SkyGuard site specific storm warning service for 14 locations: 1.Tornadoes within 3 miles with desired lead-time of 20 minutes advance notice. 2.Null Tornado warnings (when the NWS issues a tornado warning for the county, vendor will send a courtesy copy of the warning along with comments indicating if facility will be affected.) Winds of 50 mph or greater (NWS criteria is 58 mph or higher) with desired lead time of 20 minutes advance notice. 3.Any accumulating snow 1" or greater and/or any ice with 2-4 hours advance notice. 4.Hurricanes including 5 day window of movement, maximum wind gusts, expected rainfall. Includes consultation with hurricane meteorologists. 5.Consulting privileges which allow individual calls with meteorologists for weather-related questions. Method of Delivery All alerts will be delivered via SmartRAD, pager, email, cell phone (text messaging, up to 5 numbers) fax, or interface with Everbridge mass notification solution. Follow up phone call made to ensure receipt of warnings. Locations Main Campus, Memphis, TN North Clinic, Memphis, TN South Clinic, Memphis, TN Jackson Clinic, Jackson, TN Byhalia Clinic, Byhalia, MS Bolivar Clinic, Bolivar, TN Dyersburg Clinic, Dyersburg, TN Helena Clinic, Helena, AR Jonesboro Clinic, Jonesboro, AR Savannah Clinic, Savannah, TN Smithville Clinic, Smithville, MS Houlka Clinic, Houlka, MS Tremont Clinic, Tremont, MS Tupelo Clinic, Tupelo, MS Additional Components One Time Fees SmartRAD (Windows version) software with street level mapping and facility locations plotted. Licenses @ $ each = Fees 1.Real-time storm reports from local law enforcement, storm spotters displayed on SmartRad minutes after being reported. $ per month = 2.Annual cost of weather data monitoring subscription. $ per month = 3.Hurricane forecast service $ per month = THIS REQUIREMENT SHOULD BE QUOTED FOR BASE YEAR BEGINNING 3-1-10 THROUGH 9-30-10 AND FOUR OPTIONAL YEARS.
