Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.(ii) This RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38. (iii) This solicitation is a total small business set-aside, NAICS 314110, Carpet and Rug Mills, with a small business size standard of 500 employees. The National Park Service encourages the participation of small, disadvantaged, and women-owned businesses. (iv) The Government intends to issue a firm-fixed price purchase order to the responsible offeror whose total offer represents the best value for the Government. The National Park Service is seeking a qualified contractor with specialized experience with the replacement and installation of new carpeting in Theodore Roosevelt Birthplace, 28 East 20th Street, New York, NY. The work includes removal of the existing damaged carpet and the Historic reproduction of a new carpet to match the existing carpet. The installation shall be in accordance to the specifications in the Statement of Work (included below).(v) LOCATION: Gateway National Recreation Area - Theodore Roosevelt Birthplace, National Historic Site, 28 East 20th Street, New York, NY 10003.(vi) SITE VISIT: An organized site visit is scheduled for Thursday, March 4, 2010 at 10:00am. Interested parties shall respond to this announcement via email to cheryl_adamski@nps.gov, indicating their intent to attend this site visit no later than 3:00pm on March 2, 2010. Individual requests for solicitation packages will NOT be accepted. (vii) SCHEDULE of SUBMISSIONS: One (1) invoice must be submitted after successful delivery, installation, and acceptance.(viii) SCHEDULE: The actual time frame of accomplishing the project should take no longer than 90 business days. Work is slated to commence on or about March 29, 2010. (ix) DOCUMENTS TO BE INCLUDED IN QUOTE: 1. Detailed specifications including all descriptive and technical literature to be used during this project 2. A firm fixed price quote using the Bid Schedule included below.3. The date and time a CD containing all computer information used to formulate the weave and weave sequence for color and design to match the existing carpeting, will be available for delivery to the Contracting Office for evaluation.4. Applicable warranty information5. Three (3) references of the proposed services within the past five years6. Name, phone number, and address of your point of contact. (x) The Government contemplates the award of a single purchase order on a "low-price-technically-acceptable" basis. A technically acceptable business will meet or exceed the following criteria:(1) Provide a CD containing all computer information used to formulate the weave and weave sequence for color and design to match the existing carpeting.(2) Have at least five (5) years of experience performing similar services as required in this solicitation(xi) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.(xii) FAR provisions/clauses listed below apply. (xiii) The official combined synopsis/solicitation is posted at http://ideasec.nbc.gov. In order to receive the award, vendor must be registered at http://www.ccr.gov. Be prepared with your company's DUNS number (available from Dun and Bradstreet at 1-888-546-0024) and your Federal Tax Identification Number (TIN). Amendments to the solicitation will be posted in the same manner as the solicitation; it is your responsibility to monitor the website. The government reserves the right to cancel this solicitation.(xiv) Offers must be submitted via email to cheryl_adamski@nps.gov. The due date and time is 10:00am ET on Wednesday, March 10, 2010. Offers shall indicate the date and time that CDs containing all computer information used to formulate the weave sequence for color and design will be available for delivery to the Contracting Office for evaluation. CDs may be mailed to Gateway National Recreation Area, 210 New York Avenue, Staten Island, NY 10305. Attn: Cheryl Adamski, Contracting Office. (xv) Applicable Wage Determination: WD 05-2375 (Rev.-8) Statement of Work:SUMMARY OF WORKDESCRIPTIONThe work of this contract consists of: the removal of the existing damaged carpet and the Historic reproduction of a new carpet to match existing. The installation shall be in accordance to these specifications and in reasonably close conformity of the existing carpet. The work includes removal and disposal of approximately 615 SF. of damaged Carpet. The work includes all labor, materials, supplies, transportation, and disposal fees associated with removal and disposal debris/spoils. The contractor will not be responsible for moving furniture. Employees will be present while the contractor performs. The work includes; the reproduction of existing carpet and submit a computer generated design sample for approval. Contractor will inspect the existing carpet and reproduce through photographs, analyzing the existing for type of material, color, weave, thickness and backing. Carpet is located in a high traffic area and must be able to withstand the foot traffic generated by Park visitors. The replacement carpet will have a five (5) year warranty on wear and one year (1) warranty on installation. Contractor will provide a CD containing all computer information used to formulate the weave and weave sequence for color and design. All work will be done under a single contract. A. REMOVING AND INSTALLATION: a. The worksite is located in Theodore Roosevelt Birthplace, 28 East 20th Street, New York New York. Contractor shall repair any floor damage due to carpet removal, installers shall have successfully completed not less than three (3) projects of similar scope and size, must have a minimum of five (5) years experience in replacing carpet in a Historic environment and have extensive familiarity of working in a museum area. Contractor must utilize an on site lead installer with a minimum of five (5) years experience in the removal and installation of carpet in a historic environment. b. The contractor is responsible to match gauge of existing carpet, verify all dimensions of replacement quantities, and determine the amount of materials for this project. The contractor is responsible to provide all necessary labor, purchase/transport all supplies/materials and supply all equipment. The contractor is responsible for all costs associated with the removal and disposal of all excess materials, along with any refuse.c. All work shall be in accordance to applicable codes and standards including New York City Administrative code. The contractor shall comply with all applicable safety codes and standards. B. PERMITSa. Submittals: The Contractor will submit a carpet sample similar in texture to the carpet that is to be installed, to the Contracting Officer, (CO) for approval. These materials shall not be purchased until approved by the Contracting Officer.b. All work shall be complete at the time of acceptance.C. LOCATION a. Theodore Roosevelt Birthplace, 28 East 20th Street, New York, New York. D. CONTRACTOR'S USE OF PREMISES a. Construction Camp: Establishment of a camp within the park shall not be permitted.b. Contractor shall schedule all work with the Contracting Officer. c. Any on site storage of materials shall be coordinated with the Contracting Officer d. Preservation of Natural Features: Confine all operations to work limits of the project. Prevent damage to natural surroundings. Restore damaged areas, repairing or replacing shall be accomplished at no additional expense to the Government.e. Existing Utilities: Contractor shall not be allowed to use any on site utilities.E. SPECIAL CONSTRUCTION REQUIREMENTSa. Contractor shall provide a work schedule and sequence of events for approval to the Contracting Officer prior to performing any work. Work schedule shall be provided one (1) week prior to start.b. Work Restrictions: Work shall be performed between 8am and 5pm Monday through Friday, excluding Holidays, unless otherwise approved by the Contracting Officer.F. FIELD VERIFICATIONa. Field verify existing conditions and dimensions affecting the work of this contract.G. CONSTRUCTION MATERIALS - Not used.H. SALVAGED MATERIALS - a. Stockpiling of material on property may be permitted, however only with the approval of the Contract Officer and must be coordinated with the Contracting Officer Technical Reprehensive (COTR).b. Any unused new carpet will be turned over to the National Park Service.I. Period of Performance: a. Project completion shall be 90 working days this includes material delivery. Work is slated to begin on or about March 29, 2010. J. INVOICESa. One (1) invoice shall be submitted after successful delivery, installation, and acceptance.K.
