STATEMENT OF WORK MEDICAL CONTROL AND ASSOCIATED ADMINISTRATIVE SERVICES FOR THE EMERGENCY MEDICAL TECHNICIAN PROGRAM DRUG ENFORCEMENT ADMINISTRATION DEA TRAINING ACADEMY QUANTICO, VIRGINIA 22134 Background: The Drug Enforcement Administration (DEA) has a need to maintain an Operational Medical Support program to protect its personnel and support its enforcement operations and other activities. The Federal Employees Compensation Act (FECA), 5 USC 81, provides benefits to all civilian Federal employees for work-related injuries and illnesses. Courts have held that FECA provides compensation for not only the initial injury, but also additional injuries caused by negligent treatment. Further, the Due Process Clause of the Fourteenth Amendment requires the responsible government entity to provide medical care to persons who have been injured while being apprehended by the police, and the courts have found that the government has a duty to protect citizens from the actions of a person in custody when transported to a medical facility. Therefore, DEA has an interest in providing appropriate, early medical care to those who need it as a result of a DEA action. Force health protection is important to help ensure that DEA's ability to perform essential missions are not impaired and that the public safety is maintained, as required by Homeland Security Presidential Directive 7 (HSPD-7, Sections (7)(b), (8),(15) and (24). The program must include physician medical control of pre-hospital providers, provision of expert medical consultation and support services in a comprehensive, well-integrated package. Program Objectives: The Contractor shall provide continuing medical education applicable to EMT-Basic through EMT-Paramedic recertification requirements. The provision of expert medical consultation, research, and support services, including training of Special Agents and other employees in appropriate basic and advanced emergency medical skills, and physician medical consultation to pre-hospital providers. 1. Physician Qualifications: 1.1. Physician(s) shall be board certified in emergency medicine to qualify for physician pre-hospital consultation under this section. 1.1.1. Current certification is required in Advanced Cardiac Life Support (ACLS) and Advanced Trauma Life Support (ATLS) 1.1.2. Physician shall have training and an operational background in Tactical Emergency Medicine. 1.2. Medical training and continuing emergency medicine education shall be conducted by board certified specialists in emergency medicine, cardiology, general surgery, internal medicine, or related sub-specialties. 2. Administrative Tasks by the Contractor: 2.1. The Contractor shall provide a physician who serves as Director of Medical Support - (Contractor) and ensure round the clock access to this physician or his or her designee. Through this physician, The Contractor shall provide, at DEA's request, real-time medical consultation for DEA pre-hospital care providers upon request of the DEA. 2.2. The contractor shall provide consultation regarding DEA pre-hospital care capability. 2.2.1. The Contractor shall develop and submit to the DEA for review, recommended programs for skills maintenance and proficiency review, and recommended modifications or additions to existing DEA written standing orders and protocols for pre-hospital medical care. Updates or revisions of these recommendations shall be provided by the Contractor as requested by the DEA. The first review and recommendation for modifications is due no later than 120 days from the date the contract is effective. The Contractor agrees to adopt DEA's current standing orders and protocols for pre-hospital medical care. 2.2.2. The Contractor shall, at DEA's request, review current DEA guidelines for DEA pre-hospital care providers in the planning, writing, and establishment of a medical annex to the operational order in deployed operations. Updates or revisions of this review shall be provided by the Contractor as requested by DEA. 2.2.3. The Contractor shall assist, as requested by DEA, in the operation or a quality improvement process, which includes periodic 100% record review. Except as may be required by law, the Contractor shall not disclose or disseminate these records outside of the Contractor or the DEA for purposes other than quality improvement without prior DEA approval. 2.2.4. The Contractor shall provide the DEA with recommendations regarding the equipment and supplies utilized by DEA pre-hospital care providers in various missions. 2.2.5. All patient care reports will be received and maintained by the contractor. The Contractor will safeguard the reports in accordance will all applicable laws and regulations. The patient care reports remain the property of DEA and upon request the contractor will make the reports available to the DEA EMS Coordinator and or his/her designee. 3. Training and Continuing Education Provided by Contractor: 3.1. Medical Training and Continuing Education shall be provided by the Contractor or its affiliates. 3.2. A series of initial training and continuing education shall be provided annually to include by not limited to: 3.2.1. Provide via the Contractor or its affiliates, one-hundred twenty (120) hours of EMT-Basic (EMT-B) continuing education in three, forty-hour courses, for up to thirty (30) DEA EMTs per course for an annual total of ninety (90) EMTs. Continuing education shall be in accordance with National Registry of Emergency Medical Technicians (NREMT) guidelines and specifically referenced to the most current edition of the National Standard Curriculum for the EMT-B. This activity must be approved for recertification credit by the NREMT, and include three U.S. Department of Transportation National Standard EMT-Basic refresher courses. 3.2.2. Inclusive of each 40 hour training course will be the American Heart Association (AHA) Cardiopulmonary Resuscitation (CPR) Recertification for Health Care Providers course. Upon successful completion of the AHA CPR Recertification for Health Care Providers course, students will receive an AHA CPR for Health Care Providers card. 3.2.3. No less than three instructors are required for each continuing education class. 3.2.4. Course dates will be coordinated between the DEA EMS Coordinator and the Contractor. 3.2.5. Instructor-student ratio, course content, and method of instruction for these training activities will be mutually acceptable to the Contractor and the DEA. The DEA representative for emergency medicine training is the Emergency Medical Services coordinator. 3.2.6. Training and continuing education provided under this section shall be conducted at the DEA facilities or, as approved by the DEA, at facilities secured for that purpose by the Contractor. All training material shall be furnished by the Contractor. 3.2.7. All courses will be developed in conjunction with guidance provided by the DEA EMS coordinator or his designated representative. All course material will be approved by the DEA EMS coordinator and/or his designee at least two weeks prior to the beginning of the course. As part of the approval process the Contractor will submit for review to the DEA EMS coordinator and/or his designee a copy of all lectures and presentations to be presented, to include a copy of student handouts. 3.2.8. All lectures and presentations will be supported by student handouts, which will be presented to the students prior to the beginning of the lecture/presentation. 3.2.9. The DEA will retain the right to contract with any medical training provider to perform training not included in this proposal. 3.2.10. Other training providers may teach knowledge, skills, and techniques beyond the scope of practice and protocols of the DEA EMS Program. Exposure to this training does not constitute permission or authority to practice advanced levels of care. 3.2.11. All new or advanced skills, techniques, equipment, medications, and protocols will be approved by the Medical Director prior to inclusion in the DEA EMS Program Protocols and scope of practice. 3.2.12. The DEA Medical Director will accept all continuing education hours awarded by third party vendors recognized to do so by their controlling authority. 4. Place of Performance: 4.1. Medical Direction / Control work is performed offsite at a Medical Outfitters Training Consultants, LLC approved facility. 4.2. Continuing education classes are performed at the DEA Training Academy, Quantico, VA. 5. Period of Performance: The period of performance for furnishing services is from the effective date of the contract through three hundred and sixty five (365) consecutive days with four (4) successive twelve month option periods, subject to the Government's needs and the availability of funds. In the event the options are exercised, the maximum periods of performance for each option shall be as follows: Option I: 365 Consecutive Days Option II: 365 Consecutive Days Option III: 365 Consecutive Days Option IV: 365 Consecutive Days
