Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The resultant purchase order will include all applicable provisions and clauses in effect through the Federal Acquisition Circular 05-35. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement constitutes a small business competition solicitation. This synopsis, NAICS code 811219 with a SBA size standard of $7 million, is to notify contractors that the government intends to issue a Firm Fixed Price contract for (1) base year and (2) option years in accordance with FAR part 13.106 for the following statement of work, under the Simplified acquisition procedures. Any firm that believes it is capable of providing the required services as stated herein may submit a capability statement. A determination to compete this procurement based on a response to this notice is solely within the discretion of the Government. The Government reserves the right to award a contract without discussions if the Contracting Officer determines that the initial offer(s) is/are providing the Best Value and discussions are not necessary. Award will be made to the party whose quote offers the Best Value to the Government. The Government may award this contract to other than lowest price technically acceptable quote. Technical specifications and past performance is significantly more important than price. This solicitation is issued as a Request for Quotation (RFQ). Statement of Work (SOW) Service Support Agreement: This service support agreement will require the contractor to provide the following: 1. Telephone support for all ophthalmic service related problems. Return phone calls will be made within 2 hours. 2.On-site service calls for repairs of ophthalmic equipment. On-site calls to be performed within 48 hours. 3.On-site visits after hours to ensure equipment is in good working order and to address any problems that may occur. 4.Installation of parts. 5.Telephone ordering of parts. Parts to be billed to and paid for by the VA. 6.Annual Preventative Maintenance of all equipment. 7. MUST BE FULLY TRAINED CERTIFIED OPHTHALMIC TECHNICIAN. Ophthalmic Preventative Maintenance on the following equipment required: Ophthalmic Chair Ophthalmic Stand Slit Lamp Phoroptor Projector Projector Mount POC Screen & Mirror Hand Instruments Trial Lens Set Trial Frame Applanation Tonometer Tonometer Mount Lensmeter BIO High Tech Room NCT AutoRefractor Fundus Camera Power Tables Place of Performance: VAOPC Ophthalmology Clinic 1833 Boulevard Jacksonville, FL 32206 Period of Performance: Please provide a quote for each of the following years of coverage: Base year 1: 3/25/2010-9/30/2010 Option year 1: 10/01/2010-9/30/2011 Option year 2: 10/01/2011-9/30/2012 Evaluation Criteria: 1. The contractor shall demonstrate technical understanding of the statement of work. 2.Cost 3.The contractor shall provide relevant past performance for the same or similar work. CCR: Vendors must be registered in the Central Contractor Register (CCR) prior to the award of a contract. You may register by going to www.ccr.gov. You will need your Dun & Bradstreet number and banking information. Quotations Due Date: All quotations are due via email to carol.mcneill@va.gov no later than March 15, 2010 at 3:00 p.m. EST. Provisions and Clauses: SMALL BUSINESS PROGRAM REPRESENTATIONS (MAY 2004) (a)(1) The North American Industry Classification System (NAICS) code for this acquisition is 811219. (2) The small business size standard is $7 million]. (3) The small business size standard for a concern which submits an offer in its own name, other than on a construction or service contract, but which proposes to furnish a product which it did not itself manufacture, is 500 employees. (c) Definitions. As used in this provision- "Service-disabled veteran-owned small business concern"- (1) Means a small business concern- (i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and (ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran. (2) "Service-disabled veteran" means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16). "Small business concern" means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and the size standard in paragraph (a) of this provision. "Veteran-owned small business concern" means a small business concern- (1) Not less than 51 percent of which is owned by one or more veterans (as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and (2) The management and daily business operations of which are controlled by one or more veterans. "Women-owned small business concern" means a small business concern- (1) That is at least 51 percent owned by one or more women; or, in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and (2) Whose management and daily business operations are controlled by one or more women. (d) Notice. (1) If this solicitation is for supplies and has been set aside, in whole or in part, for small business concerns, then the clause in this solicitation providing notice of the set-aside contains restrictions on the source of the end items to be furnished. (2) Under 15 U.S.C. 645(d), any person who misrepresents a firm's status as a small, HUBZone small, small disadvantaged, or women-owned small business concern in order to obtain a contract to be awarded under the preference programs established pursuant to section 8(a), 8(d), 9, or 15 of the Small Business Act or any other provision of Federal law that specifically references section 8(d) for a definition of program eligibility, shall- (i) Be punished by imposition of fine, imprisonment, or both; (ii) Be subject to administrative remedies, including suspension and debarment; and (iii) Be ineligible for participation in programs conducted under the authority of the Act.
