Attachment 3 Commercial Item Clauses Attachment 2 Pricing Table Attachment 1 Statement of Work (SOW) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information attached to this notice. This announcement constitutes the only solicitation; quotations are being requested at this time. A written solicitation will not be issued. This combined synopsis/solicitation is issued as Request for Quotation (RFQ) SSA-RFQ-10-1157. The Social Security Administration is seeking quotations for Walnut Wood Plaques. This is a Small Business Set-Aside. The Statement of Work (SOW), Pricing Table, and Commercial Item Clauses for the Plaques are included in attachments #1 through #3 of this notice. The small business size standard is 500 employees. The North American Industry Classifications System (NAICS) code for this acquisition is 321999. The contract is a Requirements type contract upon which fixed price delivery orders will be placed. The life of the contract is five years from date of award. This acquisition is being conducted in accordance with the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures. The selected offerer must comply with the following commercial item terms and basis of award conditions, as well as any addenda to these clauses. The FAR clauses and provisions that apply to this acquisition are FAR 52.212-1, Instructions to Offerers-Commercial items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items. FAR 52.212-5, Contract terms and conditions required to Implement Status or Executive Orders - Commercial Items. In addition, Offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and certifications- Commercial Items. Applicable terms and conditions, FAR Clauses, and representations and certifications are contained in attachment #3 of this notice. This procurement is being issued as a 100% Total Small Business Set-Aside. Only qualified sellers may submit quotes. A quote will be considered received when the written quotation and sample product are delivered. All packages delivered to the Office of Acquisition and Grants (OAG) must be signed for. To be considered for award, offered plaques must meet all of the requirements stated in the SOW, ( attachment 1). The contractor shall also provide a firm fixed price, (completed Pricing Table (attachment 2) for the Walnut wooden Plaques for each year of the contract listed in this solicitation. The Contracting Officer will evaluate offers on the basis of information furnished by the offeror. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. The Government will award a contract resulting from this solicitation to the responsible offered whose offer conforms to the requirements of this solicitation in terms of technical acceptability, lowest price and delivery within 15 days. Shipping/ delivery must be free on board (FOB) destination CONUS (Continental U.S.). Responses to this combined synopsis/ solicitation are due no later than 4:00 Eastern Monday, March 15, 2010. Offerors may send responses via email to Phyllis.hopson@ ssa.gov. All questions regarding this RFQ must be submitted in writing to Phyllis Hopson at the email address provided previously by 4:00 Eastern Thursday, March 11, 2010. NO PHONE CALLS WILL BE ACCEPTED. Email communications shall include company name, point of contact, email address, phone and fax numbers. Offerers must be registered in the Central Contractor Registration (CCR) data base before an award can be made to them. If the offerer is not registered in the CCR, they may do so via the CCR website at http://www.ccr.gov.
