- The Stratton VA Medical Center, located at 113 Holland Ave., Albany NY 12208, is soliciting a full service firm fixed-price contract for preventative maintenance/service and on-call emergency support for a GE Healthcare VCT XT w/GT1700 64 slice CT Scan. This coverage shall include full support for the Advantage Workstation, software support, and tube coverage. The base period of performance for this contract shall begin on May 1, 2010, and shall expire on September 30, 2010. This contract will provide for one base year, and four option years that may be exercised at the discretion of the government. Contractor shall submit an official company quotation, printed on company letterhead, including the pricing per unit and in total as detailed in blocks 19-24 of the SF1449. No deviation from this pricing breakdown will be accepted. Duplicate information to this quotation should be handwritten or typewritten into blocks 19-24. In addition, the contractor is required to provide the resumes and qualifications of all personnel who may service VA equipment or be on VA premises during service. Included should be any certifications, clearances, or evidences of necessary training to perform the work as listed herein. Quotes MUST be submitted in hard copy, containing the following at a minimum: 1.The SF1449 (the first page of this solicitation packet), with all pricing filled in. Blocks 30 a, b, and c must be filled in by the contractor or an authorized representative. Original signatures must be submitted; photocopies or other duplicates will not be accepted. 2.An official quotation, as described above. 3.Qualifications of key personnel, as described above. 4.The completed Representations and Certifications Section of the solicitation packet. 5.The contact information for the member of the contractor's staff that will be administering any contract discussions, clarifications, or negotiations.
