STATEMENT OF WORK MAT REPLACEMENT FOR John J Pershing VAMC AT THE John J Pershing Medical Center Cripple Creek VA Offices Cape Girardeau CBOC 1 MARCH 2010 TABLE OF CONTENTS TITLEPAGE TABLE OF CONTENTS2 DECRIPTION OF SERVICES3 DELIVERANCE AND ACCEPTANCE CRITERIA3 TERM OF CONTRACT4 COMPENSATION AND PAYMENT4 PERSONNEL POLICY4 CONTRACTOR STAFF ROLES AND RESPONSIBILITIES5 SPECIFIC MANDATORY TASKS & DELIVERABLES5 PERFORMANCE ASSESSMENT6 BID/PRICE SCHEDULE7 Statement of Work DESCRIPTION/SPECIFICATIONS The vendor shall provide replacement of vendor owned cleaned mats at entryways throughout the facilities listed in this Statement of Work (SOW), on a weekly basis to medical facilities at VA Medical Center 1500 North Westwood Blvd. buildings 1, 2, 3, 4, 5, 6, and 8 (color: to be determined upon award); Cripple Creek VA Medical Center Offices (color: to be determined upon award); and Cape Girardeau Community Based Outpatient Clinic (CBOC) (color: to be determined upon award). Quantity and size of mats may vary depending on weather and season as needed. This includes the replacement of any damaged mats during this service contract at no additional expense to government. Westwood Blvd City: Poplar Bluff State & Zip: MO. 63901 Phone: (573) 778-4049 Fax: (573) 778-4038 Email: Timothy.Davis2@va.gov DELIVERABLES AND ACCEPTANCE CRITERIA PICK-UP/ DELIVERY TIMES AND LOCATIONS. A delivery/pick-up ticket for each location shall be given to the Government for all items. The contractor shall pick-up/deliver once a week, Monday through Friday, except for federal holidays or unscheduled closures, in which case the pick-up/delivery day shall be the next regular business day (Monday through Friday) for the following locations: 1. JOHN J PERSHING VAMC (1500 N. Westwood Blvd., Building 1,2,3,4,5,6, and 8, Poplar Bluff, Missouri). Contractor arrival time shall be between 0730 hours and 1630 hours. Phone Number is (573) 778-4049. 2. CRIPPLE CREEK VAMC OFFICES (2959-2965-2971 Kannell Blvd., Poplar Bluff, Missouri). Contractor arrival time shall be between 0800 hours and 1600 hours. Phone Number is (573) 778-4049. 3. CAPE GIRARDEAU CBOC (3051 Williams Street, Cape Girardeau, Missouri). Contractor arrival time shall be between 0800 hours and 1600 hours. Phone number is (573) 778-4049. UNSCHEDULED CLOSURE. The contractor will be notified at least 72 hours in advance of an unscheduled closure. Upon the customers request, and mutual agreement with the contractor, pick-up/deliveries may be required outside of normal hours at premium charges, if applicable. In the event of closure, the VA Medical Center EMS Supervisor will make arrangements for service. In addition, if weather conditions exist which prevents contractors delivery within the specified hours or operations, the VA Medical Center EMS Supervisor, shall be notified within one hour of the estimated time of arrival. An alternate method of pick-up and delivery of mats shall be provided by the contractor should this situation arise if feasible. Additionally, the Government, as proof of performed service, shall retain a copy of the pick-up/delivery ticket. HOLIDAYS. No pick-ups or deliveries shall be made on Saturdays, Sundays or observed Federal holidays as shown: New Years Day, January 1 Martin Luther King Day, third Monday in January Presidents Day, last Monday in February Memorial Day, last Monday in May Independence Day, July 4 Labor Day, first Monday in September Columbus Day, second Monday in October Veterans Day, November 11 Thanksgiving Day, fourth Thursday in November Christmas Day, December 25 TERM OF CONTRACT This contract is effective for one year with the possibility of four (4) option years. The period of performance for base year is from APRIL 1, 2010 through SEP 30, 2010. The contract and option years are subject to the availability of VA funds. TYPE OF CONTRACT It is the VAs intention to award a firm-fixed-price contract for the identified services for commercial items under the authority of Title 38 U.S.C. Sections 8153 to the contractor providing the best overall value to the Government. Products covered under this contract are listed in the Bid/Price Schedule. Payment method and invoicing instructions are listed within the Contract. CONTRACTOR REQUIREMENTS PERSONNEL. The contractor is responsible for all costs, including personnel costs, associated with this contract. LABOR AND MATERIALS. Contractor shall provide all labor and materials to complete service. INSURANCE. The Contractor shall, at its own expense, provide and maintain during the entire performance of this contract, at least the kinds and minimum amounts of insurance required for these services, to include worker compensation and employers liability insurance. CONTRACT MANAGER. The contractor shall provide a contract manager who shall be responsible for the performance of the work and be the point of contact. AUTHORITY. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. AVAILABILITY. The contract manager or alternate shall be available during normal duty hours 0730-1630, to meet on the John J Pershing VA Medical Center (VAMC) installation with government personnel designated by the contracting officer to discuss problem areas. After normal duty hours, the manager or designated alternate shall be available within 24 hours. CONTRACTOR STAFF, ROLES AND RESPONSIBILITIES. APPEARANCE. Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the company name and employee name. EMPLOYMENT RESTRICTION. The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest. SECURITY REQUIREMENTS. The contractor shall ensure all personnel employed in the performance of this contract, or any representative of the contractor entering the Government installation, abides by all applicable security regulations (Policy Memorandum 132-09, VA Handbook 0730). SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES 1 PROVIDE SERVICE, MATERIAL AND LABOR TO PROVIDE CLEAN FLOOR MATS FOR THE POPLAR BLUFF VAMC. TO INCLUDE THE SERVICE COMPANY PROVIDING NEW FLOOR MATS WITH THE FOLLOWING SIZES, COLORS, AND NUMBERS AS OUTLINED IN THE SCHEDULE BELOW. THIS INCLUDES THE REPLACEMENT OF ANY DAMAGED MATS DURING THIS SERVICE CONTRACT. MATS WILL BE DELIVERED AND PLACED AT ENTRYWAYS OF BUILDINGS 1, 2, 3, 4, 5, 6 AND 8. (54 MATS TOTAL) 2 PROVIDE SERVICE, MATERIAL AND LABOR TO PROVIDE CLEAN FLOOR MATS FOR THE CAPE GIRARDEAU CBOC. TO INCLUDE THE SERVICE COMPANY PROVIDING NEW FLOOR MATS WITH THE FOLLOWING SIZES, COLORS, AND NUMBERS AS OUTLINED IN THE SCHEDULE BELOW. THIS INCLUDES THE REPLACEMENT OF ANY DAMAGED MATS DURING THIS SERVICE CONTRACT. MATS WILL BE DELIVERED AND PLACED AT ENTRYWAYS OF VA OFFICE SPACES 2959-2965-2971. (8 MATS TOTAL) 3 PROVIDE SERVICE, MATERIAL AND LABOR TO PROVIDE CLEAN FLOOR MATS FOR CRIPPLE CREEK VA OFFICES. TO INCLUDE THE SERVICE COMPANY PROVIDING NEW FLOOR MATS WITH THE FOLLOWING SIZES, COLORS, AND NUMBERS AS OUTLINED IN THE SCHEDULE BELOW. THIS INCLUDES THE REPLACEMENT OF ANY DAMAGED MATS DURING THIS SERVICE CONTRACT. MATS WILL BE DELIVERED AND PLACED AT ENTRYWAYS OF THE CAPE GIRARDEAU COMMUNITY BASED OUTPATIENT CLINIC. The contractor service requirements are summarized into performance objectives that relate directly to the items required. The performance standards briefly describe the minimum acceptable levels of service required for this contract. Performance ObjectivePerformance StandardSOW ParaPerformance Threshold Clean Floor MatsCarpets and mats are clean, free of stains and discoloration.1.195% of all facilities receiving periodic services are without customer complaints for the reporting period. QUALITY CONTROL. The contractor shall develop and maintain a quality program to ensure custodial services are performed in accordance with commonly accepted commercial practices. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective or unacceptable levels of performance as described in paragraph 2. QUALITY ASSURANCE. The Government may do assessments of the Contractors performance. The Contractor shall have an opportunity to respond to assessments. GOVERNMENT REMEDIES. The contracting officer shall follow FAR 52.212.4, Contract Terms and Conditions-Commercial Items (July, 2008), for contractors failure to perform satisfactory services or failure to correct non-conforming services.
