- PRESOLICITATION NOTICE The W.G. (Bill) Hefner VA Medical Center, 1601 Brenner Avenue, Salisbury, NC has a need for a Residential Mental Health and Substance abuse facility. The contractor shall provide services to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs Medical Center, Salisbury, NC herein after referred to as the (VA). The contractor shall provide to the VA, Community Based Health Care for Homeless Veterans (HCHV), in accordance with all terms and conditions, provisions and requirements. The contractor shall furnish each veteran authorized care under this contract with the following basic services: a. Room and Board, including at least two (2) nutritionally adequate meals a day, 7 days a week and availability of nutritious snacks between meals and bedtime for those requiring or desiring additional food, when it is not medically contraindicated, b. Laundry facilities for residents to do their own laundry or to have laundry done, c. Therapeutic and Rehabilitative Services determined to be needed by the individual patient in a plan developed by the contractor with input from the patient and the VA HCHV case manager. Services which the contractor must be able to furnish include: (1) Structured group activities, including physical activities as appropriate. (2) Instruction in and assistance with health and personal hygiene, (3) Monitoring of medications, including locked drawers for storage of medication. (4) Supportive social services, in collaboration with the HCHV case managers, or other contract resources. (5) Individual professional counseling, including counseling on self care skills, adaptive coping skills, and as appropriate, vocational rehabilitation counseling, in collaboration with the VA's Compensation Work Therapy program. (6) Assistance in learning and development of responsible living patterns to achieve a more adaptive level of psychosocial functioning upgraded social skills and improved personal relationships. (7) Support for an alcohol/drug abuse free lifestyle by maintaining a drug and alcohol free environment. (8) Assistance in learning, testing and internalizing knowledge of the illness/recovery process for homelessness. (9) Assist with arranging transportation to care at the VA Medical Center and other appointments. This requirement is unrestricted. The NAICS Code is 623220. Size Standard is $6.5 million. Contract period will be Date of Award through September 30, 2010 with (2) Two Year Option to extend the contract. The procurement will be conducted utilizing the commercial item procedures of FAR 12 in conjunction with FAR Part 13. Solicitation packages will be available on or about March 23, 2010. Proposals will be due on or about April 26, 2010. No hard copy solicitations will be provided. This is a 100% Total Small Business Set-Aside Procurement.
