Description
Temporary Waiver forms This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-10-Q-0026. This procurement is 100% small business set-aside under NAICS code 446110 with a standard business size of $7.0M. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-40, effective March 23, 2010. Contractor shall provide a firm, fixed-price hourly rate (all inclusive to include travel and per diem) for 600 hours, May 1 2010 - April 30 2011. Estimated start date is May 1, 2010. The closing date for receipt of quotes is April 9 2010. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on April 9 2010. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. Qualified vendors will review the following and submit applicable information. STATEMENT OF WORK Pharmacy Services White Cloud Indian Health Station, White Cloud KS BACKGROUND AND PURPOSE A part-time contract pharmacist will be needed whose mission is to provide accessible, full-scope, quality pharmacy services to eligible Indian patients at the White Cloud Indian Health Station by staffing the pharmacy department at the White Cloud Indian Health Station. Pharmacy services will be available to eligible patient at the White Cloud Indian Health Station. The staff of the station and the pharmacist shall work in close cooperation in order to meet the needs of the patient. The primary purpose of this contract is to provide the patients seen at the White Cloud Indian Health Station quality pharmacy services. The contractor shall hold a current state license in pharmacy. STATEMENT OF WORK Pharmacy services shall be provided at the White Cloud Indian Health Station. Onsite services shall be available on clinic days (Mondays, Wednesday, and Fridays), and on Tuesdays and Thursdays, when the contractor will be available to answer phone calls, process refills, and perform any basic administrative duties associated with pharmacy workflow, such as ordering/restocking supplies and assisting with Point of Sale issues. Work will be scheduled in a manner mutually agreed upon by the contractor and the facility director. Clinical services: The services will include the following: 1. Process, fill, and dispense prescriptions per provider's orders, assuring appropriateness of drug therapy. 2. Verify that patients understand their medication and appropriate outcomes of their drug therapy. 3. Assure availability, preparation and control of medications. 4. Provide drug information, drug therapy consultation and staff education regarding drug therapy. 5. Provide health information and disease prevention counseling relation to drug use and preventive drug therapy. 6. Perform a variety of professional tasks of average technical difficulty in fulfillment of the department's basic patient care and service functions. 7. Exercises independent judgment in accomplishment of a variety of professional tasks. Judgment failure could seriously affect the health status of one more individuals. 8. No supervisory duties are required. The Contractor shall provide five (5), eight (8) hour days of pharmacy service per week as needed. Services shall be scheduled in a manner that is mutually agreed upon between the Contractor and the service unit. A non-personal service fixed price contract will be made from this solicitation. GOVERNMENT FURNISHED PROPERTY, FACILITIES, AND SERVICES - The contractor will perform duties in the White Cloud Indian Health Station pharmacy, using the equipment and supplies provided. - Administrative and professional support is available to Indian Health Service personnel. This will include but not be limited to personal computer, copy machines, telefax machines, medical references, and telephone services. - Initial and ongoing orientation to the White Cloud Indian Health Station, clinic policy and procedures related to performance of duties outlined in the work statement and the Indian community served. - Use of government furnished facility space, equipment, supplies, and personnel are for the provision of care and services set forth in the contract only. CONTRACTOR-PROVIDED EQUIPMENT The Indian Health Service does not anticipate a need for contractor furnished equipment. However, the Chief Pharmacist may allow any instrument or piece of equipment specifically preferred or requested by the contractor after approval. EVALUATION AND REPORTING REQUIREMENTS Allow the government to evaluate the quality of professional and administrative services provided, but retain no control over the medical, professional, aspects of the services rendered (e.g., professional judgment). The service unit shall provide an ongoing method of monitoring and evaluating patient services set forth in this contract. PROVIDER/CONTRACTOR QUALIFICATIONS AND REQUIREMENTS The Contractor shall meet the following: - Hold a current, unrestricted state professional license in pharmacy from any of the fifty (50) United State or the Commonwealth of Puerto Rico. - Notify the White Cloud Indian Health Station of any changes in contract privileges at any non-IHS facilities. - Have no work or health restrictions interfering with the performances of pharmacy services. - Speak, understand, read and write English fluently. Communicate with sufficient clarity to be understood by the patients and staff of the White Cloud Indian Health Station. - Possess sufficient initiative, interpersonal relationship skill, and culturally sensitivity such that he/she can relate constructive to patients from the Native American community. - Standards of personal hygiene and grooming such that his/her image as a pharmacist is compatible with the expectation of the White Cloud Indian Health Station staff and Indian community. - Indemnify the Government for any liability producing act or omission of the Contractor, it employees and agents occurring during contract performance. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) completed "Temporary Waiver of Character Investigation" and "Declaration for Federal Employment" form (see attachment) (2) current medical insurance (3) two past performance references to include the contact name phone number, contract number, company name, brief description of project (4) Dun and Bradstreet Number (note: Contractors must be registered in the Central Contractor Registration to be eligible for award. This can be done at http://www.ccr.gov ). Failure to do so will deem the quote as unacceptable. The provisions of 52.212-2, Evaluation - Commercial Items (Jan 1999) applies to this acquisition. The Government intends to award one from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price; (2) Qualifications; and (3) Past Performance. Award will be made to the lowest priced, technically acceptable offeror. Quotes shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (Nov 2007) or indicate certifications in ORCA at https://orca@bpn,gov. The estimated quantity is 600 hours for a one-year period. The estimated start of award is May 1 2010. F) Proof of current medical insurance in the appropriate amount of $1,000,000 per occurrence and $3,000,000 aggregate. G) Two past performance references.
