Description

II. This is modification # 3 to contract F09CC00432. The purpose is increase the funding for additional services in performance of repairing the bridges at the National Zoological Park SI Project # 0933108. III. The Contractor shall perform all the duties outlined in the attached SI revised Scopes of Work letters dated December 1, 2009, January 19, February 9, 3,10,12, 23, 12, and 22 and the contractor's letters of proposal dated December 14, 2009, February 2, 9, 17, and 24, 2010 March 8, 9, February 26, and March 8, 2010 respectively. The changes are as follows: PCN # 4, Additional concrete repairs to east abutment @ bridge # 3- $55,549.17 PCN # 11, Additional work and bridge 2 soffit (McMullan Field report)- $40,391.91 PCN # 13, Additional Concrete Repairs to the west abutment & Bridge 3- $50,445.52 PCN # 14, Additional deck repairs & bridge 3 (McMullan Field Report)- $37,821.36 PCN # 15, Replacement of 20 broken balustrade @ Bridge 2- $21,973.11 PCN # 16, Additional conduit installation, trenching, backfilling, tree prot- $60,013.52 PCN # 17, Additional milling and asphalt replacement @ bridge 2- $25,486.98 PCN # 18, Voids underneath sidewalks and curbs @ bridge 3- $6,237.06 PCN # 19, Sandblasting existing face walls @bridge 2- $49,217.97 In addition to the additional work listed above, the completion date for this contract has been extended 30 days to July 31, 2010. The total fixed price of these changes is $347,136.60 increasing the total value of this contract from $1,147,828.92 to $1,494,965.52
